PT-141, known generically as bremelanotide, is a synthetic cyclic peptide originally derived from the melanocortin peptide Melanotan II. It is the first approved pharmacological agent that addresses sexual dysfunction through the central nervous system (CNS), distinguishing it from conventional treatments such as PDE-5 inhibitors, which act primarily on the vascular system. In the United States, bremelanotide is FDA-approved under the brand name Vyleesi for the treatment of hypoactive sexual desire disorder (HSDD) in premenopausal women.
PT-141 acts as a melanocortin receptor agonist, binding primarily to the MC3R and MC4R receptor subtypes in the brain. These receptors are involved in the regulation of sexual behavior, appetite, and energy balance. By activating these receptors in the hypothalamus and other brain regions, PT-141 enhances sexual arousal and desire without directly affecting the cardiovascular system.
