What is PT-141?

PT-141, known generically as bremelanotide, is a synthetic cyclic peptide originally derived from the melanocortin peptide Melanotan II. It is the first approved pharmacological agent that addresses sexual dysfunction through the central nervous system (CNS), distinguishing it from conventional treatments such as PDE-5 inhibitors, which act primarily on the vascular system. In the United States, bremelanotide is FDA-approved under the brand name Vyleesi for the treatment of hypoactive sexual desire disorder (HSDD) in premenopausal women.

Mechanism of Action

PT-141 acts as a melanocortin receptor agonist, binding primarily to the MC3R and MC4R receptor subtypes in the brain. These receptors are involved in the regulation of sexual behavior, appetite, and energy balance. By activating these receptors in the hypothalamus and other brain regions, PT-141 enhances sexual arousal and desire without directly affecting the cardiovascular system.

Indications and Usage

PT-141 is primarily used for:

Hypoactive sexual desire disorder (HSDD) in premenopausal women (FDA-approved)

Sexual dysfunction in individuals who do not respond adequately to PDE-5 inhibitors

The approved route of administration is a subcutaneous injection (under the skin), administered approximately 45 minutes before anticipated sexual activity. The recommended dose is 1.75 mg. Use should not exceed once every 24 hours or more than eight times per month.

In off-label contexts – such as bodybuilding or anti-aging medicine – higher doses are sometimes used, which is not medically recommended and carries additional safety risks.

Side Effects

PT-141 can cause a range of side effects. The most commonly reported include:

Nausea – occurring in over 40% of users
Flushing (sensation of warmth, skin redness)
Headache
Injection site reactions
Hyperpigmentation of the skin (with prolonged use)
Transient increase in blood pressure

Due to the risk of a temporary rise in blood pressure, PT-141 is contraindicated in individuals with known cardiovascular disease or uncontrolled hypertension.

Contraindications and Interactions

PT-141 should not be used in individuals with:

Uncontrolled arterial hypertension
Hypersensitivity to bremelanotide or any of its components

Clinically relevant drug interactions include:

Naltrexone and other opioid antagonists (altered pharmacokinetics)
Indomethacin and other NSAIDs (increased risk of blood pressure elevation)
Other melanocortin agonists
