What is Rosuvastatin?

Rosuvastatin is a prescription medication belonging to the class of drugs known as statins (HMG-CoA reductase inhibitors). It is primarily used to lower elevated cholesterol levels in the blood and is considered one of the most potent statins available. It is marketed under the brand name Crestor, among various generic formulations.

Indications

Rosuvastatin is prescribed for the following conditions and risk factors:

  • Hypercholesterolaemia: elevated LDL cholesterol levels (so-called bad cholesterol)
  • Mixed dyslipidaemia: a lipid metabolism disorder involving elevated triglycerides and low HDL cholesterol
  • Familial hypercholesterolaemia: a hereditary condition causing severely elevated cholesterol levels
  • Prevention of heart attacks, strokes, and other cardiovascular events in patients at elevated risk

Mechanism of Action

Rosuvastatin works by inhibiting the enzyme HMG-CoA reductase, which is responsible for cholesterol synthesis in the liver. By blocking this enzyme, less cholesterol is produced internally. In response, the liver increases the number of LDL receptors on its surface, which then remove excess LDL cholesterol from the bloodstream. The overall effect is a significant reduction in LDL cholesterol levels, a moderate decrease in triglycerides, and a slight increase in HDL cholesterol (the so-called good cholesterol).

Dosage

The usual starting dose is 5 to 10 mg once daily, which may be increased up to 40 mg depending on therapeutic response and tolerability. Rosuvastatin is taken once daily, with or without food, and should be taken at the same time each day. Dosing is always determined individually by a healthcare provider.

Special Patient Populations

  • Elderly patients: No dose adjustment is generally required, but increased vigilance for side effects is recommended.
  • Renal impairment: A maximum dose of 20 mg per day is recommended for patients with moderate kidney impairment.
  • Asian patients: Due to genetic differences in drug metabolism, a lower starting dose is often recommended.

Side Effects

Rosuvastatin is well tolerated by most patients. Possible side effects include:

  • Common: Headache, dizziness, nausea, constipation, muscle pain (myalgia)
  • Uncommon: Muscle weakness or inflammation (myopathy), elevated liver enzymes
  • Rare: Rhabdomyolysis (severe muscle breakdown) – this is a medical emergency requiring immediate medical attention
  • Slight increase in blood glucose and elevated risk of type 2 diabetes in predisposed individuals

Patients should contact their doctor immediately if they experience unusual muscle pain, weakness, or cramps, especially when accompanied by fever or dark-colored urine.

Drug Interactions

Rosuvastatin may interact with various medications and substances:

  • Anticoagulants (e.g., warfarin): increased bleeding risk possible
  • Ciclosporin: significantly increased rosuvastatin blood levels; dose adjustment required
  • Antacids: may reduce the absorption of rosuvastatin
  • Niacin and fibrates: increased risk of muscle-related adverse effects

Contraindications

Rosuvastatin must not be used in patients with:

  • known hypersensitivity to the active substance
  • active liver disease
  • Pregnancy and breastfeeding (cholesterol is essential for fetal development)
  • severe renal impairment

Treatment Context

Rosuvastatin is typically used as part of a comprehensive treatment plan that also includes lifestyle modifications such as a heart-healthy diet, regular physical activity, weight management, and smoking cessation. Medication alone is rarely sufficient – a healthy lifestyle is essential for long-term therapeutic success.
