A rubber dam (also called a dental dam or cofferdam) is a thin, flexible sheet -- traditionally made of latex -- that is used in dentistry to isolate one or more teeth from the rest of the oral cavity during treatment. The sheet is stretched over the tooth or teeth in question, secured with metal clamps, and held in place by a frame. This creates a clean, dry, and bacteria-controlled working field, which is essential for many dental procedures.

When is a Rubber Dam Used?

The rubber dam is used in a variety of dental settings. The most common applications include:

  • Root canal treatment (Endodontics): Isolating the tooth prevents saliva and oral bacteria from contaminating the root canal system. It also protects the patient from accidentally swallowing or inhaling small instruments or irrigation fluids.
  • Composite resin fillings: Moisture and saliva can significantly compromise the bonding of modern tooth-coloured fillings. A dry working field ensures better adhesion and a longer-lasting restoration.
  • Teeth whitening (Bleaching): The rubber dam protects the gums and oral mucosa from contact with potentially irritating bleaching agents.
  • Paediatric dentistry: The rubber dam provides an additional layer of safety for children by preventing swallowing or inhalation of small instruments or materials.

Components and Application

The rubber dam system consists of several components:

  • Rubber dam sheet: A thin latex sheet (or a latex-free alternative for patients with latex allergies) that is stretched over the teeth.
  • Rubber dam clamp: A metal clamp that anchors the sheet firmly around the tooth.
  • Rubber dam frame: A U-shaped metal or plastic frame that holds the sheet taut during the procedure.
  • Rubber dam punch: A specialised tool used to create small holes in the sheet through which the tooth or teeth are exposed.

Placing a rubber dam typically takes only a few minutes and is well tolerated by most patients. Some patients may find the sensation unusual at first, but usually adapt quickly.

Advantages of the Rubber Dam

The use of a rubber dam offers significant benefits for both the patient and the dental professional:

  • Safety: Prevents accidental swallowing or inhalation of small instruments, materials, and irrigation solutions.
  • Hygiene: Reduces bacterial contamination of the treatment area by isolating it from saliva.

In rare situations -- such as severely limited mouth opening or unusual anatomical features -- placement of a rubber dam may be more challenging, but is rarely impossible.
