What does selective mean in medicine?

The term selective derives from the Latin word seligere (to choose) and in medicine and pharmacology describes the property of a drug, procedure or mechanism to act specifically on defined target structures. A selective action means that only certain receptors, cells, tissues or organs are targeted, while other structures are largely spared.

Importance in pharmacology

In pharmacology, selectivity is a central quality criterion for medicinal drugs. The more selective a substance is, the more precisely it can be used therapeutically and, as a rule, the lower the risk of unwanted side effects.

  • Selective serotonin reuptake inhibitors (SSRIs): These antidepressants act specifically on the serotonin transporter while having minimal effect on other neurotransmitter systems.
  • Selective beta-blockers: So-called cardioselective beta-blockers act preferentially on beta-1 receptors in the heart and less on beta-2 receptors in the lungs.
  • Selective COX-2 inhibitors: These analgesics specifically inhibit the enzyme cyclooxygenase-2, which is involved in inflammatory processes, while largely protecting the gastric mucosa.

Selectivity in oncology

In cancer medicine, selectivity plays a particularly important role. Targeted therapies are directed against specific molecules or genetic alterations in tumor cells. This enables more effective treatment while reducing the burden on healthy tissue compared to conventional chemotherapy.

Selectivity in diagnostics

Selectivity is also a key concept in diagnostics. A selective test reliably detects the target feature (e.g., a specific biomarker or pathogen) without being interfered with by similar substances or structures. In laboratory medicine, a distinction is made between sensitivity and specificity (selectivity) of a test.

Selectivity in surgery and radiation therapy

In surgical and radiation therapy procedures, selective refers to a targeted intervention that treats only the diseased tissue. Examples include selective arterial embolization, in which specific blood vessels are deliberately occluded, and stereotactic radiosurgery, which irradiates tumor tissue with high precision while protecting surrounding healthy structures.

Clinical relevance

The principle of selectivity is relevant in virtually all medical disciplines. A high degree of selectivity in drugs, procedures or diagnostic methods contributes to:

  • Minimizing side effects
  • Increasing therapeutic efficacy
  • Reducing the burden on patients
  • Improving treatment outcomes
