What is Semaglutide?

Semaglutide is a synthetic compound belonging to the class of GLP-1 receptor agonists (glucagon-like peptide-1 receptor agonists). It mimics the action of the naturally occurring gut hormone GLP-1, which is released after meals and plays a central role in regulating blood sugar levels and satiety. Semaglutide is approved for the treatment of type 2 diabetes mellitus and, at higher doses, for the treatment of obesity and overweight.

Mechanism of Action

Semaglutide binds to the GLP-1 receptor and triggers several important effects in the body:

  • Blood sugar reduction: It stimulates insulin release from the pancreas in a glucose-dependent manner and simultaneously inhibits the secretion of glucagon, a hormone that raises blood sugar.
  • Delayed gastric emptying: It slows down digestion, reducing post-meal blood sugar spikes.
  • Appetite suppression: Semaglutide acts on areas of the brain (hypothalamus) responsible for hunger and satiety, leading to a sustained reduction in appetite.
  • Weight loss: The combination of reduced appetite and lower caloric intake results in significant body weight reduction.

Indications

Type 2 Diabetes Mellitus

In type 2 diabetes, semaglutide is used when diet, exercise, and lifestyle changes alone are insufficient to achieve adequate blood sugar control. It can be used as monotherapy or in combination with other antidiabetic agents such as metformin.

Obesity and Overweight

At a higher dose, semaglutide is also approved for the treatment of obesity (BMI of 30 kg/m² or above) or overweight (BMI of 27 kg/m² or above) in patients with at least one weight-related comorbidity. Clinical trials have shown an average weight reduction of up to 15 percent of body weight.

Cardiovascular Protection

Clinical evidence shows that semaglutide significantly reduces the risk of heart attack, stroke, and cardiovascular death in patients with type 2 diabetes and established cardiovascular disease.

Dosage Forms and Administration

Semaglutide is available in several formulations:

  • Subcutaneous injection (weekly): As a prefilled pen under the brand names Ozempic (for diabetes) and Wegovy (for obesity), semaglutide is injected once weekly under the skin.
  • Oral tablet (daily): Under the brand name Rybelsus, semaglutide is available as a daily tablet – making it the first orally available GLP-1 receptor agonist worldwide.

The dose is typically increased gradually (dose titration schedule) to improve tolerability and minimize side effects.

Side Effects

The most common side effects involve the gastrointestinal tract:

  • Nausea and vomiting
  • Diarrhea or constipation
  • Abdominal pain and bloating

These symptoms occur most frequently at the beginning of treatment and often improve after a few weeks. Less common but more serious side effects may include:

  • Pancreatitis (inflammation of the pancreas)
  • Diabetic retinopathy (worsening of pre-existing eye disease in diabetes)
  • Gallbladder problems
  • Increased heart rate (tachycardia)

Contraindications

Semaglutide must not be used in patients with:

  • Known hypersensitivity to the active substance
  • Personal or family history of medullary thyroid carcinoma
  • Multiple endocrine neoplasia type 2 (MEN2)
  • Pregnancy or breastfeeding
