What is Simvastatin?

Simvastatin is a cholesterol-lowering medication belonging to the drug class known as statins. It is used to reduce elevated blood lipid levels – particularly elevated LDL cholesterol (often referred to as “bad cholesterol”) – and to lower the risk of heart attack, stroke, and other cardiovascular diseases. Simvastatin has been well established for decades and is one of the most widely prescribed medications worldwide.

Indications

Simvastatin is primarily used in the following conditions:

  • Hypercholesterolaemia: Elevated blood cholesterol levels that cannot be adequately controlled through diet alone.
  • Mixed dyslipidaemia: Disorders of lipid metabolism involving elevated triglycerides and reduced HDL cholesterol.
  • Primary prevention: Reducing the risk of cardiovascular events in patients at high risk, such as those with diabetes mellitus or arterial hypertension.
  • Secondary prevention: Lowering the risk of further heart attacks or strokes following a previous cardiovascular event.

Mechanism of Action

Simvastatin acts as a competitive inhibitor of the enzyme HMG-CoA reductase (3-hydroxy-3-methylglutaryl coenzyme A reductase). This enzyme represents the rate-limiting step in the body's own cholesterol synthesis, which takes place primarily in the liver. By inhibiting this enzyme, less cholesterol is produced in the liver. In response, the liver generates more LDL receptors on its surface, which leads to increased uptake of LDL cholesterol from the bloodstream. The result is a significant reduction in blood LDL cholesterol levels, typically by 25–45 %.

Dosage and Administration

Simvastatin is taken as an oral tablet. The usual dose ranges from 10 mg to 40 mg per day. Evening administration is generally recommended, as the body's endogenous cholesterol production occurs mainly at night. In exceptional cases, the dose may be increased to 80 mg; however, this is rarely recommended today due to an increased risk of muscle-related side effects. The exact dosage is determined individually by the prescribing physician.

Important Notes on Administration

  • Simvastatin should not be taken together with grapefruit juice, as it inhibits the breakdown of the medication and can dangerously increase its blood concentration.
  • The medication should be taken regularly and at the same time each day.
  • Discontinuation should only be done in consultation with a physician.

Side Effects

Like all medications, simvastatin can cause side effects. Most patients tolerate it well, but possible side effects include:

  • Muscle complaints (myopathy): Muscle pain, heaviness, or weakness are the most common side effects. In rare cases, a serious muscle injury known as rhabdomyolysis can occur, which requires immediate medical attention.
  • Liver enzyme changes: An increase in liver enzyme levels is possible; regular monitoring is recommended.
  • Gastrointestinal complaints: Nausea, abdominal pain, or constipation may occur.
  • Headache and dizziness have occasionally been reported.
  • Increased diabetes risk: Long-term studies have shown a slightly increased risk of developing type 2 diabetes.

Drug Interactions

Simvastatin has numerous clinically relevant drug interactions. Special caution is required when taken simultaneously with:

  • Certain antibiotics (e.g., erythromycin, clarithromycin)
  • Antifungal agents (e.g., itraconazole, ketoconazole)
  • HIV protease inhibitors
  • Amiodarone (a cardiac medication)
  • Fibrates (other lipid-lowering agents)
  • Ciclosporin (an immunosuppressant)

All of these substances can slow the breakdown of simvastatin and increase the risk of muscle-related side effects. It is therefore important to inform the prescribing physician about all medications being taken.

Contraindications

Simvastatin must not be used in:

  • Known hypersensitivity to the active substance
  • Active liver disease or persistently elevated liver enzymes
  • Pregnancy and breastfeeding (teratogenic potential)
  • Concomitant use of certain substances (e.g., strong CYP3A4 inhibitors)
