What Is the Sleep Quality Index?

The sleep quality index is a standardized assessment tool used to evaluate a person's subjective perception of sleep quality. The most widely recognized version is the Pittsburgh Sleep Quality Index (PSQI), developed in 1989 by Buysse and colleagues. It is used across sleep medicine, psychiatry, and general practice to detect sleep disturbances, monitor their severity, and evaluate treatment outcomes.

The index assesses multiple dimensions of sleep over a four-week period, resulting in a total score that indicates whether sleep quality is good or poor.

Structure and Components

The Pittsburgh Sleep Quality Index consists of 19 self-rated questions grouped into seven components:

  • Subjective sleep quality: How does the individual perceive their own sleep?
  • Sleep latency: How long does it take to fall asleep?
  • Sleep duration: How many hours of actual sleep are obtained per night?
  • Habitual sleep efficiency: The ratio of time actually sleeping to time spent in bed.
  • Sleep disturbances: Frequency of issues such as waking up, pain, or nightmares.
  • Use of sleep medication: Frequency of using medications to promote sleep.
  • Daytime dysfunction: How much daytime functioning is impaired due to poor sleep.

Each component is scored from 0 to 3. The total score ranges from 0 to 21. A score of more than 5 is generally considered indicative of poor sleep quality and a potentially clinically significant sleep disorder.

Areas of Application

The sleep quality index is used across a wide range of medical and research settings:

  • Diagnosis and monitoring of insomnia (difficulty falling or staying asleep)
  • Supporting diagnostics in depression, anxiety disorders, and chronic pain conditions
  • Assessing sleep problems in older adults and chronically ill patients
  • Sleep and health research in clinical trials
  • Evaluating treatment success, such as with Cognitive Behavioral Therapy for Insomnia (CBT-I)

Administration and Scoring

The PSQI is a self-report questionnaire completed by the patient independently. It is simple to use and typically takes only 5 to 10 minutes to complete. Scoring is performed by healthcare professionals using a standardized scoring algorithm. In addition to the PSQI, other instruments are also used to measure sleep quality, such as the Epworth Sleepiness Scale (ESS) for daytime sleepiness and the Insomnia Severity Index (ISI) specifically for insomnia symptoms.

Clinical Significance

Poor sleep has far-reaching consequences for overall health. It increases the risk of cardiovascular disease, metabolic disorders, depression, and impaired immune function. The sleep quality index is a vital tool for identifying affected individuals early and initiating targeted treatment. Regular assessment of sleep quality is therefore important in both primary care and specialist medicine.
