What Is a Spinal Cord Injury?

A spinal cord injury (SCI) refers to damage to the spinal cord, the main bundle of nerves that runs from the brain down through the vertebral column. Damage to this delicate tissue can cause permanent or temporary impairments in motor function, sensation, and the control of internal organs.

Spinal cord injuries are classified as either complete -- where there is no motor or sensory function below the level of injury -- or incomplete -- where some residual function is preserved below the injury level.

Causes

Spinal cord injuries can result from sudden traumatic events or from non-traumatic medical conditions.

  • Tetraplegia (quadriplegia): Paralysis of all four limbs, resulting from injuries to the cervical (neck) region
  • Paraplegia: Paralysis of the lower limbs, resulting from injuries to the thoracic or lumbar region

Diagnosis

Diagnosis of a spinal cord injury is based on a combination of clinical neurological examination and imaging studies.

  • Neurological assessment: Evaluation of motor and sensory function using standardized tools such as the ASIA Impairment Scale (AIS)
  • X-ray: Initial assessment of vertebral fractures and misalignment
  • Computed Tomography (CT): Detailed imaging of bony structures
  • Magnetic Resonance Imaging (MRI): Gold standard for evaluating the spinal cord, intervertebral discs, and surrounding soft tissues

Treatment

Treatment of a spinal cord injury is complex and requires a multidisciplinary team including neurologists, neurosurgeons, rehabilitation specialists, and nursing staff.

Acute Management

  • Immobilization: Immediate spinal stabilization at the scene to prevent further injury
  • Intensive care monitoring: Ensuring adequate breathing and circulation
  • Surgical intervention: Decompression of the spinal cord and stabilization of the spine with implants if needed
  • Medical therapy: Management of swelling and inflammation

Rehabilitation

Rehabilitation is the cornerstone of long-term treatment, aiming to restore lost functions as much as possible and promote the independence of the patient.
