What Is a Split Night?
A split night (also called a split-night study) is a specialized procedure in sleep medicine in which a diagnostic polysomnography – a comprehensive measurement of bodily functions during sleep – and a CPAP titration (adjustment of the breathing mask pressure) are conducted in a single night. The night is divided into two segments: during the first part, the sleep disorder is diagnosed, and during the second part, treatment is initiated immediately.
When Is a Split Night Used?
A split night is primarily used when a patient is suspected of having obstructive sleep apnea (OSA), a condition characterized by repeated interruptions of breathing during sleep that can lead to serious health consequences. The split-night approach allows faster patient care and avoids the need for a second inpatient stay.
Typical indications include:
- Loud snoring with observed breathing pauses
- Excessive daytime sleepiness (hypersomnia)
- Suspected severe obstructive sleep apnea
- High Apnea-Hypopnea Index (AHI) on initial screening
How a Split Night Works
First Part: Diagnosis
During the first segment of the night – typically the first two to four hours – the patient undergoes comprehensive sleep monitoring. Measured parameters include:
- Brain activity (EEG – electroencephalogram)
- Eye movements (EOG – electrooculogram)
- Muscle activity (EMG – electromyogram)
- Heart rhythm (ECG)
- Airflow and respiratory effort
- Blood oxygen saturation (pulse oximetry)
- Snoring sounds and body position
Second Part: CPAP Titration
Once the first part of the night reveals findings that are sufficiently severe – typically an AHI of 15 or more per hour – the patient is fitted with a CPAP mask (Continuous Positive Airway Pressure) for the remainder of the night. The sleep technician adjusts the air pressure to the optimal level that effectively eliminates the breathing pauses. This process is known as titration.

Limitations and Disadvantages
The split night is not suitable for all patients. If the sleep apnea is not sufficiently severe during the first part of the night, or if the findings are inconclusive, the remaining time may not be enough for a reliable CPAP titration. In such cases, a separate titration night is required. Additionally, a too-short titration phase may result in an imprecise determination of the optimal therapeutic pressure.
