What is Spotting?

Spotting refers to very light vaginal bleeding that is not part of a regular menstrual period. The amount of blood is minimal – often just a few drops or a slight discoloration on underwear. Spotting can occur at different points in the menstrual cycle, such as around ovulation, just before or after a period, or during pregnancy.

Causes

Spotting can have a wide range of causes, from completely harmless to medically significant. Identifying the underlying reason is important for appropriate management.

Hormonal Causes

  • Ovulation spotting: Light bleeding at mid-cycle when an egg is released
  • Hormonal imbalances: Fluctuations in estrogen and progesterone levels
  • Starting or changing hormonal contraception (e.g., birth control pills, hormonal IUD)
  • Perimenopause: Irregular bleeding due to declining hormone levels

Pregnancy-Related Causes

  • Implantation bleeding: Light spotting occurring approximately 6–12 days after fertilization
  • Early miscarriage
  • Ectopic pregnancy: A medical emergency requiring immediate attention
  • Placenta previa or other placental complications

Gynecological Causes

  • Cervical polyps or cervicitis (inflammation of the cervix)
  • Endometriosis
  • Uterine fibroids (benign tumors of the uterus)
  • Genital tract infections (e.g., chlamydia)
  • Cervical cancer or other malignancies (rare but important to exclude)

Other Causes

  • Mechanical irritation, e.g., after sexual intercourse (contact bleeding)
  • High stress levels or intense physical activity
  • Thyroid disorders

Symptoms and Appearance

Spotting is typically characterized by the following features:

  • Very small amount of blood – often just a few drops or a faint stain
  • Color ranges from bright red to pinkish or brownish (older blood)
  • Short duration (usually a few hours to a couple of days)
  • Little to no accompanying pain

If spotting is accompanied by severe pain, fever, dizziness, or significantly heavier bleeding, medical attention should be sought immediately.

Diagnosis

To determine the cause of spotting, a healthcare provider will typically perform the following:

  • Medical history: Questions about the menstrual cycle, contraception, pregnancy status, and associated symptoms
  • Gynecological examination: Visual inspection of the vagina and cervix
  • Ultrasound (sonography): Assessment of the uterus and ovaries
  • Pregnancy test (hCG level)
  • Cervical swab and laboratory tests to rule out infections or cellular abnormalities
  • Hormone testing if a hormonal cause is suspected

Treatment

Treatment of spotting depends entirely on the underlying cause:

  • Hormonal causes: Adjustment of contraception or hormone therapy
  • Infections: Antibiotic or antiviral treatment
  • Polyps or fibroids: Surgical removal if symptomatic
  • Endometriosis: Hormonal therapy or surgical intervention
  • Pregnancy complications: Close medical monitoring, possibly hospitalization
  • Suspected malignancy: Further diagnostic workup and oncological treatment

Harmless spotting – such as ovulation spotting – generally does not require treatment and only needs to be monitored.

When to See a Doctor

A single, brief, and painless episode of spotting is often not a cause for concern. However, medical evaluation is recommended if:

  • spotting occurs regularly or repeatedly
  • it happens during pregnancy
  • it is accompanied by pain, fever, or other symptoms
  • it occurs after menopause
  • the amount of bleeding is increasing
