SS-31, also known by the drug name Elamipretide or the designations MTP-131 and Bendavia, is a synthetically produced tetrapeptide molecule belonging to the class of Szeto-Schiller peptides (SS-peptides). This class of peptides was developed by scientists Hazel Szeto and Peter Schiller and is specifically designed to selectively target the inner mitochondrial membrane, where it exerts protective effects. SS-31 consists of four amino acids (D-Arg-Dmt-Lys-Phe-NH2) and is considered one of the most promising candidates in the field of mitochondrial medicine.
The central mechanism of action of SS-31 is based on its ability to selectively bind to cardiolipin – a unique phospholipid found exclusively in the inner mitochondrial membrane that is essential for the function of the respiratory chain and for energy production (ATP synthesis).

In clinical trials, SS-31 (as Elamipretide) is being investigated for heart failure and ischemia-reperfusion injury (tissue damage following restoration of blood flow, e.g., after a heart attack). Preclinical data demonstrated a reduction in infarct size and improved cardiac function.
Due to the high energy demands of kidney cells, mitochondrial dysfunction can contribute to acute kidney injury (AKI). SS-31 is being investigated for its nephroprotective (kidney-protecting) effects.
Mitochondrial damage is a well-known feature of Alzheimer disease, Parkinson disease, and other neurodegenerative conditions. Animal models showed that SS-31 can exert neuroprotective effects and slow cognitive decline.
In rare primary mitochondrial myopathies (genetically determined diseases of the mitochondria), SS-31 is being studied as a potential therapy, as it can improve mitochondrial function independently of the specific genetic mutation involved.
Since oxidative stress and mitochondrial functional decline are considered major drivers of biological aging, SS-31 is also being investigated in the context of geroscience (the science of aging). Animal studies showed improvements in muscle function, endurance, and overall vitality in older animals.
In clinical trials, SS-31 (Elamipretide) is typically administered as a subcutaneous injection or intravenous infusion. Since oral bioavailability of the molecule is insufficient, oral formulations are not a focus of clinical development. Standardized dosing recommendations for clinical use outside of trials do not currently exist, as SS-31 is not yet available as an approved drug.

Serious adverse events were rarely reported in early-stage studies. Since long-term safety and efficacy have not yet been fully established, SS-31 should only be used within the context of clinical trials or under medical supervision.
SS-31 has been investigated in multiple Phase I, Phase II, and Phase III clinical trials. Notable studies include the PROGRESS-HF trial (in heart failure with reduced ejection fraction) and studies in primary mitochondrial myopathies (PMM). Although some trials delivered mixed primary endpoints, biomarker data and secondary endpoints continue to demonstrate biological activity and clinical potential. Research is ongoing and the molecule is considered one of the leading mitochondrial drug candidates in clinical development.
