What is Stand-by Therapy?

Stand-by therapy (also written as standby therapy or emergency self-treatment) is a precautionary medical strategy for travelers visiting malaria-endemic regions. Before departure, a doctor prescribes suitable antimalarial medication that the traveler carries and self-administers if fever and malaria-like symptoms develop and no medical care is promptly accessible. It is not a replacement for professional medical treatment but serves as a bridging measure until proper care can be obtained.

When is Stand-by Therapy Used?

Stand-by therapy is recommended when:

  • Travelers visit high-risk malaria regions where adequate medical facilities are not readily available.
  • Regular malaria chemoprophylaxis is not feasible or not desired.
  • Travelers spend extended periods in remote areas with limited access to medical care.
  • Symptoms such as fever, chills, headache, or muscle pain develop between 7 days and several months after exposure in a risk area.

Mechanism of Action and Medications Used

Stand-by therapy relies on antimalarial drugs that inhibit the development of the Plasmodium parasite in the blood or eliminate it. Depending on the destination and local resistance patterns, different active substances are used:

  • Artemether/Lumefantrine (e.g., Riamet®): A combination of two compounds with different mechanisms; considered a well-tolerated first-line option for many regions.
  • Atovaquone/Proguanil (e.g., Malarone®): Inhibits mitochondrial function in the parasite; particularly suitable where resistance to other agents is present.
  • Mefloquine: Acts on the blood schizogony cycle of the parasite; less commonly used due to potential neuropsychiatric side effects.

The appropriate medication is always chosen individually by a physician, based on travel destination, resistance patterns, existing health conditions, and possible drug interactions.

Dosage and Administration

The exact dosage depends on the specific preparation and the body weight of the traveler. Key principles include:

  • The medication should be taken only when fever of 38 °C or above is present along with malaria-suspicious symptoms.
  • Self-treatment does not replace a medical consultation -- a doctor should be seen as soon as possible.
  • A thorough pre-travel medical consultation at a travel medicine clinic is essential to explain usage, dosage, and to rule out contraindications.
  • The medication must always be kept accessible and stored correctly (cool, dry, protected from light).

Possible Side Effects

Side effects vary depending on the active substance used. Common side effects include:

  • Artemether/Lumefantrine: Headache, dizziness, sleep disturbances, abdominal pain, nausea.
  • Atovaquone/Proguanil: Nausea, vomiting, abdominal pain, headache.
  • Mefloquine: Sleep disturbances, dizziness, mood changes, and rarely severe neurological reactions.

In the event of severe or unexpected reactions, medical help should be sought immediately.

Important Notes and Limitations

Stand-by therapy is not a substitute for malaria chemoprophylaxis or general protective measures such as insect repellents, mosquito nets, and appropriate clothing. It should only be carried and used on medical recommendation. Special caution is required for pregnant women, children, and individuals with certain pre-existing conditions, who require individualized guidance.
