What is a Stress Fracture?

A stress fracture (also known as a fatigue fracture) is a small, hairline crack in a bone that develops not from a single traumatic event, but from repeated mechanical stress over time. When the bone is subjected to more load than it can repair and remodel, micro-damage accumulates and a stress fracture forms. Common sites include the metatarsal bones of the foot, the tibia (shinbone), and the fibula.

Causes

Stress fractures typically result from a combination of excessive physical activity and inadequate recovery. Key causes and risk factors include:

  • Overtraining: Sudden increases in training intensity or duration
  • Inappropriate footwear or hard training surfaces
  • Biomechanical abnormalities such as flat feet or leg length discrepancies
  • Nutritional deficiencies, particularly low calcium and vitamin D, which reduce bone density
  • Osteoporosis or other conditions that weaken bone
  • Female Athlete Triad: the combination of low energy availability, menstrual irregularities, and low bone density

Symptoms

Stress fracture symptoms typically develop gradually and may initially be mistaken for general muscle soreness. Common signs include:

  • Activity-related pain that worsens during exercise and eases with rest
  • Localized tenderness directly over the affected bone
  • Swelling or mild bruising around the injury site
  • In advanced cases: pain at rest or even during daily activities

Diagnosis

A stress fracture is diagnosed through clinical examination combined with imaging studies:

  • X-ray: Often normal in early stages; changes may become visible after 2-3 weeks
  • MRI (Magnetic Resonance Imaging): The gold standard for early detection, even when X-rays appear normal
  • Bone scintigraphy: Detects increased bone metabolism at the fracture site
  • CT scan: Useful for assessing complex anatomical locations or pre-surgical planning

Treatment

Treatment depends on the severity and location of the stress fracture:

Conservative Treatment

  • Rest and offloading: Reducing or stopping the activity that caused the injury
  • Orthopaedic aids: Specialised footwear, insoles, or walking boots to reduce stress on the bone
  • Physiotherapy: Targeted exercises to rebuild strength and correct movement patterns
  • Pain management: Anti-inflammatory medications such as ibuprofen (under medical guidance)
  • Nutritional support: Ensuring adequate intake of calcium and vitamin D to promote bone healing

Surgical Treatment

Surgery is rarely required but may be necessary in high-risk locations (e.g., femoral neck or tarsal navicular) or when conservative treatment fails. Internal fixation using screws or intramedullary nails may be performed to stabilise the fracture.

Healing Time and Prognosis

Most stress fractures heal within 6 to 12 weeks with appropriate rest and management, depending on the location and severity of the injury. The prognosis is generally excellent when treatment guidelines are followed. Returning to activity too soon significantly increases the risk of a complete fracture or recurrence.

Prevention

The following strategies can significantly reduce the risk of developing a stress fracture:

  • Gradual progression of training load and intensity
  • Using well-cushioned, properly fitting footwear
  • Incorporating regular rest days and recovery periods into training schedules
  • Maintaining a balanced diet rich in calcium and vitamin D
  • Addressing biomechanical issues with the help of a specialist
