Medical suction (also called aspiration or suctioning) is a clinical procedure in which a vacuum or negative pressure system is used to remove unwanted fluids, secretions, blood, mucus, or tissue debris from the body. It is applied across many medical fields, including emergency medicine, intensive care, surgery, anaesthesia, and nursing.

Indications and Applications

Suction is used in a wide range of clinical situations:

Airway suctioning: Removal of mucus, saliva, or vomit from the mouth, throat, or airways to maintain a clear airway.
Intraoperative suction: During surgical procedures, blood and body fluids are cleared from the operative field.
Wound suction: After surgery or in chronic wounds, wound secretions are removed to support healing (e.g., vacuum-assisted closure).
Liposuction: A cosmetic-surgical procedure for removing subcutaneous fat tissue.
Neonatal suction: Immediately after birth, the mouth and nose of a newborn may be suctioned if necessary.
Pleural drainage: Removal of fluid or air from the pleural cavity (the space between the lung and chest wall).

How Does Suction Work?

During a suction procedure, a catheter or probe is inserted into the target area. A motorised or manual suction device generates negative pressure, drawing unwanted substances into a collection container. Devices range from simple hand pumps to stationary electric suction units or portable emergency devices.

Procedure and Preparation

The preparation required for suctioning varies depending on the clinical context:

Selection of the appropriate catheter or probe (size, material)
Sterile or clean technique depending on the clinical situation
Patient information and consent where possible
Monitoring of vital signs, especially during airway suctioning

Risks and Complications

Like any medical procedure, suction carries potential risks:

Mucosal injury caused by the catheter
Hypoxia (oxygen deficiency) if airway suctioning is prolonged
Infection risk if non-sterile technique is used
Vagal reactions such as bradycardia (slowed heart rate) due to stimulation of the vagus nerve
Atelectasis (collapsed lung segments) from excessive negative pressure

To minimise these risks, suctioning should always be performed by trained medical personnel.

Suction in Nursing Care

In nursing, suctioning is an essential component of care for ventilated or tracheostomised patients. Nursing staff perform regular airway suction to maintain airway patency and prevent pneumonia. Strict hygiene standards must be observed throughout the procedure.
