What are Sulfonylureas?

Sulfonylureas are a class of oral antidiabetic drugs used to lower blood glucose levels in people with type 2 diabetes mellitus. They are among the oldest and most widely used medications for this condition. Well-known active substances in this group include glibenclamide, glimepiride, and gliclazide.

Mechanism of Action

Sulfonylureas work by blocking ATP-sensitive potassium channels in the insulin-producing beta cells of the pancreas. This causes depolarization of the cell membrane, which triggers an influx of calcium ions. As a result, the beta cells are stimulated to release insulin -- regardless of the current blood glucose level.

Indications

Sulfonylureas are used in the following situations:

  • Type 2 diabetes mellitus when dietary changes and physical activity alone are insufficient
  • Patients who cannot tolerate or have contraindications to metformin
  • As part of combination therapy with other antidiabetic agents

Dosage and Administration

The dosage depends on the specific active ingredient and the individual blood glucose profile of the patient. Sulfonylureas are generally taken once daily, usually before or with a meal. The exact dosage should always be determined by a healthcare professional.

Side Effects

The most important and most common side effect of sulfonylureas is hypoglycemia (low blood sugar), since insulin secretion is stimulated independently of blood glucose levels. Other possible side effects include:

  • Weight gain due to elevated insulin levels
  • Gastrointestinal complaints such as nausea or abdominal pain
  • Rarely: allergic skin reactions
  • In rare cases: disturbances in blood cell counts

Contraindications and Precautions

Sulfonylureas must not be used in the following cases:

  • Type 1 diabetes (as functioning beta cells are no longer present)
  • Severe kidney or liver impairment
  • Pregnancy and breastfeeding
  • Known hypersensitivity to the active substance

Special caution is required in elderly patients, as the risk of hypoglycemia is increased.

Drug Interactions

Sulfonylureas can interact with numerous other medications. Blood glucose-lowering effects may be enhanced by non-steroidal anti-inflammatory drugs (NSAIDs), certain antibiotics, and alcohol. Corticosteroids, thiazide diuretics, and sympathomimetics may reduce their blood glucose-lowering effect.
