What are impacted teeth?

Impacted teeth are teeth that have failed to fully erupt into their expected position in the dental arch. They remain partially or completely embedded within the jawbone or gum tissue and can cause a range of dental problems if left untreated. The most commonly affected teeth are the upper canines and the wisdom teeth.

Causes of tooth impaction

Several factors can lead to tooth impaction:

Lack of space in the dental arch
Early loss of primary (baby) teeth, altering available space
Genetic predisposition and developmental anomalies
Abnormal jaw development
Supernumerary (extra) teeth blocking eruption
Cysts or tumors in the jaw area

Diagnosis

Impacted teeth are typically identified through dental X-rays. A panoramic X-ray (OPG) is the standard diagnostic tool, while a cone beam computed tomography (CBCT) scan may be used to determine the exact three-dimensional position, depth, and angulation of the impacted tooth. Early diagnosis greatly improves treatment outcomes.

The surgical exposure procedure

Surgical exposure of an impacted tooth is a routine oral and maxillofacial surgery procedure, typically performed on an outpatient basis under local anesthesia. Sedation or general anesthesia may be considered for anxious patients or in complex cases.

Steps of the procedure

Planning: A personalized treatment plan is developed based on imaging results.
Incision: The gum tissue overlying the impacted tooth is carefully cut open.
Bone removal: If the tooth is embedded in bone, a small amount of bone tissue is removed to access the tooth crown.
Attachment: A small orthodontic bracket or gold chain is bonded to the exposed crown, which will later be used to apply gentle traction.
Wound closure: The gum is sutured closed, with the wire or chain extending to the outside for orthodontic access.

Open vs. closed exposure technique

There are two main surgical approaches:

Open exposure: The flap of gum tissue is folded back and left open, keeping the tooth visible and accessible for direct orthodontic movement.
Closed exposure: After bonding the bracket, the gum is sutured back over the tooth. Orthodontic traction is applied below the gum surface.

Treatment after exposure

Following the surgical procedure, orthodontic treatment begins. Braces and wires are used to gradually guide the exposed tooth into its correct position within the dental arch. This process can take several months to a few years and requires regular follow-up appointments with the orthodontist.

Recovery and aftercare

Mild swelling, bruising, and discomfort in the treated area are expected after the procedure and typically resolve within a few days. The following measures support healing:

Taking prescribed pain relievers and antibiotics (if recommended)
Applying ice packs to the outside of the cheek to reduce swelling
Resting and avoiding hard or chewy foods
