What Is the Symphysis-Fundus Distance?

The symphysis-fundus distance (SFD), also commonly referred to as fundal height, is a routine clinical measurement performed during pregnancy. It refers to the distance in centimetres between the symphysis pubis (the pubic bone) and the fundus uteri (the uppermost point of the uterus). This simple measurement provides valuable information about fetal growth and overall pregnancy progression.

How Is the Measurement Performed?

The symphysis-fundus distance is measured by a midwife or physician during a prenatal visit. The pregnant person lies on their back, and a measuring tape is used to measure from the upper edge of the pubic bone to the top of the uterus.

  • The measurement is painless and non-invasive.
  • It is performed routinely from approximately 20 weeks of gestation onwards.
  • As a general guideline, the SFD in centimetres roughly corresponds to the number of weeks of gestation.

Normal Values and Interpretation

As a reference, from 20 weeks of gestation the symphysis-fundus distance should approximately equal the gestational age in weeks, with an accepted variation of ±2 to 3 centimetres. For example, at 28 weeks of gestation, an SFD of approximately 26 to 30 cm would be considered within the normal range.

Elevated SFD

A larger-than-expected SFD may indicate:

  • Macrosomia: The baby is larger or heavier than expected for the gestational age.
  • Polyhydramnios: An excess amount of amniotic fluid.
  • Multiple pregnancy: Twins or more.
  • An abnormal fetal lie, such as a transverse position.

Reduced SFD

A smaller-than-expected SFD may indicate:

  • Intrauterine growth restriction (IUGR): The baby is growing more slowly than expected.
  • Oligohydramnios: Insufficient amniotic fluid.
  • Deep engagement of the fetal head in the pelvis.

Clinical Significance

The symphysis-fundus distance is a simple, cost-effective, and quick screening tool used in routine prenatal care. Abnormal measurements are not a cause for immediate alarm but serve as an indication for further investigation, particularly ultrasound examination (sonography), which provides a more detailed assessment. It is important to note that the measurement can be influenced by factors such as the body mass index (BMI) of the mother, the position of the baby, or variability between practitioners.

Comparison with Ultrasound

While the SFD offers a rapid clinical estimate, ultrasound provides significantly more precise information about fetal growth, amniotic fluid volume, and placental location. Both methods are complementary and together form an important part of comprehensive prenatal care.
