What is Pancreatic Cancer?

Pancreatic cancer, medically known as pancreatic carcinoma, is a malignant disease of the pancreas -- a vital gland located in the upper abdomen that produces both digestive enzymes and hormones such as insulin. Pancreatic cancer is considered particularly dangerous because it frequently causes no noticeable symptoms until it has reached an advanced stage.

Early Symptoms

In the early stages, pancreatic cancer often produces no symptoms or only vague, non-specific complaints. Possible early warning signs include:

  • Upper abdominal pain, sometimes radiating to the back
  • Unintentional weight loss without an obvious cause
  • Loss of appetite and general weakness
  • Nausea and digestive problems
  • Newly diagnosed or difficult-to-control diabetes mellitus

Advanced Symptoms

As the tumor grows, more pronounced symptoms may develop:

  • Jaundice (icterus): Yellowing of the skin and whites of the eyes caused by a backup of bile. This is especially common when the tumor is located in the head of the pancreas, where it can obstruct the bile duct.
  • Dark urine and pale stools: Accompanying signs of jaundice due to bile duct obstruction.
  • Severe itching (pruritus): Caused by bile salts accumulating in the skin.
  • Persistent abdominal and back pain: Due to the tumor invading surrounding nerves and organs.
  • Abdominal swelling (ascites): Fluid accumulation in the abdominal cavity in advanced disease.
  • Fatigue and exhaustion: Pronounced tiredness and reduced performance.

Symptoms by Tumor Location

The symptoms of pancreatic cancer depend significantly on where in the pancreas the tumor is located:

  • Head of the pancreas (most common location): Early-onset jaundice due to compression of the bile duct.
  • Body and tail of the pancreas: Symptoms typically appear later, often only when the tumor is larger or has already spread. Pain and weight loss tend to dominate.

Because the symptoms of pancreatic cancer are often non-specific and can also occur with less serious conditions, it is important to consult a doctor promptly if you experience persistent or combined complaints such as unexplained weight loss, upper abdominal pain, or jaundice. Early diagnosis significantly improves treatment options and outcomes.

Diagnosis

Several diagnostic methods are available to investigate suspicious symptoms:

  • Abdominal ultrasound as an initial imaging tool
  • Computed tomography (CT) or magnetic resonance imaging (MRI) for detailed tumor assessment
  • Endoscopic ultrasound (EUS) for close-up imaging of the pancreas
  • Blood tests, including the tumor marker CA 19-9
  • Biopsy for tissue sampling and pathological examination

Treatment

Treatment of pancreatic cancer depends on the stage of the disease and the overall health of the patient. Options include:

  • Surgery (resection): If the tumor has not yet spread, surgical removal is the only potentially curative option. The most common procedure is the Whipple operation (pancreaticoduodenectomy).
  • Chemotherapy: Used both as adjuvant therapy after surgery and for inoperable tumors.
  • Radiation therapy: Used in combination, particularly for locally advanced tumors.
  • Palliative care: To relieve symptoms such as pain, jaundice, or digestive problems.
