What is TRBA 250?

TRBA 250 (Technische Regel für Biologische Arbeitsstoffe 250, or Technical Rule for Biological Agents 250) is a German occupational health and safety regulation designed to protect employees in healthcare and welfare services from infection by biological agents. It provides concrete guidance for implementing the requirements of the German Biological Agents Ordinance (Biostoffverordnung, BioStoffV) and establishes binding measures to prevent needlestick injuries, cut injuries, and other percutaneous exposures.

The regulation applies to employers, works councils, occupational safety specialists, and all employees who handle sharp or pointed medical instruments, including physicians, nurses, and medical-technical assistants.

Purpose and Scope

TRBA 250 applies to activities involving biological agents in healthcare facilities, including:

Hospitals and clinics
Medical and dental practices
Nursing homes and outpatient care services
Emergency medical services and accident and emergency departments
Diagnostic laboratories

Its primary goal is to reduce the risk of infection from bloodborne pathogens such as Hepatitis B (HBV), Hepatitis C (HCV), and HIV to a minimum.

Key Requirements and Protective Measures

Risk Assessment

Employers are required to conduct a risk assessment in which all activities involving biological agents are systematically evaluated. This assessment must be documented and regularly updated to reflect current working conditions and new information.

Use of Safety-Engineered Devices

A central element of TRBA 250 is the mandatory use of safety-engineered sharps devices (also called safety devices or safe sharp instruments). These include:

Needles with automatic needle-retraction or shielding mechanisms
Retractable safety lancets
Scalpels with blade-retraction protection
Needleless infusion systems

The use of such devices is mandatory unless there are well-founded reasons to the contrary, such as a medical contraindication or genuine technical impossibility.

Organisational Measures

In addition to technical measures, TRBA 250 prescribes organisational protective measures, including:

Regular instruction and training of all employees
Provision of appropriate disposal containers for sharps (so-called sharps containers)
Prohibition of two-handed recapping of needles
Clear procedures for handling contaminated waste

Personal Protective Equipment (PPE)

TRBA 250 specifies when and what personal protective equipment must be worn, for example:

Protective gloves when handling body fluids
Surgical or respiratory masks in cases of aerosol exposure
Safety goggles or face shields when there is a risk of splashing

Vaccination

Employers are obliged to offer healthcare employees a free Hepatitis B vaccination. Vaccination is considered an essential complementary protective measure alongside technical and organisational controls.

Procedure After a Needlestick Injury

TRBA 250 also defines the procedure to follow after an accident. In the event of a needlestick injury or other contact with potentially infectious material, the following immediate measures are recommended:

Immediately encourage blood flow from the wound (gentle pressure)
Thoroughly rinse the wound under running water
Disinfect the injured area
Report the incident immediately to a supervisor and the occupational health physician
Document the incident as a workplace accident
Initiate Post-Exposure Prophylaxis (PEP) if required (e.g., in cases of HIV exposure risk)

Legal Framework

TRBA 250 is developed by the Committee on Biological Agents (Ausschuss für Biologische Arbeitsstoffe, ABAS) at the German Federal Ministry of Labour and Social Affairs (BMAS) and published in the Joint Ministerial Gazette. It has the status of a technical rule with presumption of conformity: compliance with TRBA 250 is treated as equivalent to fulfilling the requirements of the Biological Agents Ordinance.

The regulation also implements the European Council Directive 2010/32/EU (the Sharps Injuries Directive) into German national law, which mandates the protection of healthcare workers from needlestick and sharps injuries in the hospital and healthcare sector.
