What is Trimipramine?

Trimipramine is a medication belonging to the class of tricyclic antidepressants (TCAs). It is primarily used to treat depression but also has anxiolytic (anti-anxiety) and sedative (calming) effects. Known under brand names such as Stangyl and Surmontil, trimipramine is one of the older antidepressants still in clinical use today, valued especially for its pronounced sleep-promoting properties.

Indications

Trimipramine is used in the treatment of:

  • Depression, particularly when associated with anxiety and agitation
  • Anxiety disorders and panic attacks
  • Sleep disturbances associated with depressive illness
  • Chronic pain syndromes (as adjunct therapy)
  • Withdrawal symptoms in certain addictive disorders (off-label use)

Mechanism of Action

Trimipramine inhibits the reuptake of norepinephrine and serotonin into presynaptic nerve terminals. This increases the availability of these neurotransmitters in the synaptic cleft, thereby improving mood. Compared to other TCAs, the reuptake inhibition by trimipramine is relatively weak. However, it is characterized by a strong blockade of histamine H1 receptors, which accounts for its pronounced sedative effect. Trimipramine also blocks muscarinic acetylcholine receptors and alpha-1 adrenergic receptors.

Dosage

The dosage of trimipramine is determined individually by the treating physician. Typical dosage ranges include:

  • Outpatient treatment: 50–150 mg per day, divided into several doses
  • Inpatient treatment: up to 300 mg per day in severe cases
  • Sleep disturbances: low doses of 25–75 mg in the evening

The dose is usually increased gradually to improve tolerability. Elderly patients typically receive lower doses.

Side Effects

Like all tricyclic antidepressants, trimipramine can cause various side effects. Common side effects include:

  • Drowsiness and sedation (especially at the beginning of therapy)
  • Dry mouth and constipation
  • Weight gain
  • Dizziness and orthostatic hypotension (drop in blood pressure upon standing)
  • Blurred vision
  • Cardiac arrhythmias (especially at higher doses)
  • Urinary retention

Rare but serious side effects include seizures and cardiac conduction disturbances. Overdose carries a significant risk of severe cardiac complications.

Drug Interactions

Trimipramine can interact with various other medications:

  • MAO inhibitors: Concomitant use is contraindicated due to the risk of a life-threatening serotonin syndrome
  • Other sedative substances (e.g., benzodiazepines, alcohol): enhanced sedation
  • Antiarrhythmic drugs: increased risk of cardiac arrhythmias
  • Anticholinergic medications: additive anticholinergic side effects

Contraindications

Trimipramine should not be used in patients with:

  • Acute myocardial infarction or severe cardiac arrhythmias
  • Narrow-angle glaucoma
  • Urinary retention
  • Concurrent treatment with MAO inhibitors
  • Known hypersensitivity to trimipramine

Important Notes

Trimipramine is a prescription-only medication and must only be taken under medical supervision. Treatment should not be discontinued abruptly, as this may lead to withdrawal symptoms. Special caution is warranted in elderly patients, patients with cardiovascular disease, and in adolescents and young adults, as the risk of suicidal thoughts may be increased during the first weeks of treatment.
