What Are Tympanostomy Tubes?

Tympanostomy tubes (also called ear tubes, grommets, or ventilation tubes) are tiny cylindrical devices inserted through a small incision in the eardrum (tympanic membrane). They create a permanent opening between the outer ear canal and the middle ear, allowing continuous ventilation, preventing fluid accumulation, and equalizing pressure.

Why Are Tympanostomy Tubes Used?

The most common indication is otitis media with effusion (glue ear), a condition in which thick fluid accumulates in the middle ear without signs of acute infection. Additional indications include:

  • Recurrent acute otitis media (3 or more episodes within 6 months, or 4 or more within a year)
  • Chronic middle ear effusion with hearing loss
  • Eustachian tube dysfunction
  • Barotrauma from diving or air travel

Causes of the Underlying Conditions

The conditions leading to tube placement usually have multiple contributing factors:

  • Eustachian tube dysfunction: The Eustachian tube connects the middle ear to the back of the throat and regulates pressure. In young children, it is shorter and more horizontal, making it more susceptible to dysfunction and infection.
  • Enlarged adenoids: Adenoid tissue near the Eustachian tube opening can obstruct ventilation.
  • Allergies and respiratory infections: Frequent upper respiratory infections or allergies increase inflammation and the risk of middle ear problems.

The Procedure: How Are Tubes Inserted?

The procedure is called a myringotomy with tympanostomy tube insertion and is one of the most commonly performed surgical procedures in children. The typical steps include:

  • General anesthesia is usually used for children; local anesthesia may be sufficient for adults.
  • A small incision (myringotomy) is made in the eardrum.
  • Any accumulated fluid is suctioned out.
  • The tube is placed into the incision, held in position by its flanged shape.
  • The entire procedure typically takes only a few minutes.

How Long Do Tubes Stay in Place?

There are two main types of tympanostomy tubes:

  • Short-term tubes (grommets): These remain in the eardrum for approximately 6 to 18 months before falling out naturally as the eardrum heals.
  • Long-term tubes (T-tubes): Designed to stay in place for several years, these usually require surgical removal.

Benefits and Expected Outcomes

Most patients experience significant improvement following tube insertion:

  • Improved hearing through better sound conduction
  • Reduced frequency and severity of ear infections
  • Enhanced speech and language development in young children
  • Relief from pressure, pain, and a sensation of fullness in the ear

Risks and Possible Complications

Tympanostomy tube insertion is considered a safe and routine procedure. However, possible complications include:

  • Ear discharge (otorrhea): Fluid may drain through the tube; this is common and generally harmless but may require treatment if infected.
  • Premature extrusion: The tube may fall out earlier than intended.
  • Persistent perforation: Rarely, the eardrum does not close completely after the tube falls out.
  • Tympanosclerosis: Calcium deposits may form on the eardrum.
  • Risk of water entering the ear; earplugs are often recommended during swimming and bathing.

Tympanostomy Tubes in Children

Children are the most common patient group. Because the Eustachian tube in young children is not yet fully mature, they are particularly prone to middle ear problems. In children with chronic middle ear effusion, tympanostomy tubes can significantly improve hearing and speech development. The decision to proceed with surgery should always be made in consultation with an ear, nose, and throat (ENT) specialist and the child's caregivers.
