What is Ultrasound Diagnostics?

Ultrasound diagnostics, also known as sonography or ultrasonography, is a non-invasive medical imaging technique that uses high-frequency sound waves to create real-time images of structures inside the human body. Because it does not use ionizing radiation, it is considered very safe and is routinely used in pregnancy, pediatrics, and a wide range of clinical specialties. Ultrasound is one of the most widely used diagnostic tools in modern medicine.

How Does Ultrasound Work?

An ultrasound device consists of a handheld probe called a transducer, which emits high-frequency sound waves into the body and detects the echoes that bounce back from different tissue boundaries. A computer processes these echoes and converts them into two-dimensional or three-dimensional images displayed on a monitor in real time.

The sound waves used in medical ultrasound typically range from 2 to 20 megahertz (MHz). Higher frequencies provide greater image detail but have less tissue penetration, while lower frequencies are better suited for imaging deeper structures.

Clinical Applications

Ultrasound is used across a broad range of medical disciplines:

  • Abdominal organs: The liver, gallbladder, pancreas, spleen, kidneys, and bladder can all be effectively evaluated.
  • Thyroid and neck: Nodules, cysts, and enlargement of the thyroid gland are reliably detected.
  • Heart (echocardiography): Cardiac structure, valve function, and blood flow can be assessed in detail.
  • Blood vessels (Doppler ultrasound): Blood flow velocity, arterial stenosis, and venous thrombosis can be evaluated.
  • Pregnancy: Fetal development, position, placental location, and amniotic fluid levels are routinely monitored.
  • Musculoskeletal system: Tendons, muscles, joints, and ligaments can be examined for injuries or inflammation.
  • Breast: Used as a complementary tool to mammography for further evaluation of suspicious findings.

How is an Ultrasound Examination Performed?

Depending on the area being examined, some preparation may be required. For abdominal ultrasound, patients are often asked to fast for several hours beforehand to reduce bowel gas and improve image quality. A clear water-based gel is applied to the skin to ensure optimal sound transmission between the transducer and the body surface. The transducer is then gently moved over the skin while real-time images appear on the monitor.

An ultrasound examination typically takes between 5 and 30 minutes, depending on the clinical question and the area being examined. It is painless and requires no recovery time.

Specialized Ultrasound Techniques

Doppler Ultrasound

Doppler ultrasound uses the Doppler effect to measure the speed and direction of blood flow within vessels. It is particularly useful for detecting vascular conditions such as deep vein thrombosis, arterial stenosis, or circulatory insufficiency. Color-coded duplex sonography overlays color on the image to visually represent blood flow direction and velocity.

3D and 4D Ultrasound

Modern ultrasound systems can generate three-dimensional (3D) images and even real-time volumetric images (4D ultrasound). This technology is widely used in obstetrics to provide detailed visualization of the developing fetus.

Endoscopic Ultrasound

Endoscopic ultrasound (EUS) involves inserting a probe into body cavities such as the esophagus, stomach, or rectum to obtain high-resolution images of nearby structures. This is particularly valuable for evaluating the pancreas, bile ducts, and gastrointestinal tract.

Contrast-Enhanced Ultrasound (CEUS)

Contrast-enhanced ultrasound (CEUS) involves the intravenous injection of microbubble contrast agents to enhance the visualization of blood flow within organs and tumors. It improves the characterization of lesions and is an increasingly important tool in oncology and hepatology.

Limitations

Despite its versatility, ultrasound diagnostics has certain limitations. Bone and air-filled structures – such as the lungs and bowel loops – strongly reflect or absorb sound waves, making them difficult or impossible to image adequately. In patients with obesity, image quality may be reduced due to increased tissue depth. Additionally, the diagnostic accuracy of ultrasound is highly dependent on the skill and experience of the operator performing the examination.
