Thyroid values are blood laboratory results that provide information about the function of the thyroid gland. The thyroid is a butterfly-shaped gland located in the neck that produces hormones regulating metabolism, the cardiovascular system, the nervous system, and many other bodily functions. Regular monitoring of thyroid values helps detect and treat conditions such as an overactive or underactive thyroid at an early stage.
TSH (thyroid-stimulating hormone) is the most important screening value for thyroid function. It is produced by the pituitary gland and controls the thyroid's hormone production. An elevated TSH level may indicate an underactive thyroid (hypothyroidism), while a low TSH level may point to an overactive thyroid (hyperthyroidism). The normal range is generally between 0.4 and 4.0 mU/L, though this may vary slightly by laboratory.
fT3 is the biologically active thyroid hormone that acts directly on the body's cells. It regulates energy metabolism, heart rate, and body temperature, among other functions. The normal range is typically between 3.1 and 6.8 pmol/L. Elevated fT3 levels are often found in hyperthyroidism, while low levels may indicate hypothyroidism or severe systemic illness.
fT4 is the precursor to fT3 and is produced directly by the thyroid gland. In the body, fT4 is converted into the active fT3 as needed. The typical normal range is between 12 and 22 pmol/L. fT4 is often measured together with TSH to get a complete picture of thyroid function.

An elevated TSH level suggests that the thyroid is producing too few hormones. The pituitary gland responds by increasing TSH output to stimulate the thyroid. Common causes include Hashimoto thyroiditis, iodine deficiency, or thyroid damage following surgery or radiation therapy.
A low TSH level can be a sign of an overactive thyroid. Common causes include Graves' disease, autonomous thyroid nodules, or an excessive dose of thyroid hormone medication.
Changes in fT3 or fT4 with a normal TSH may point to rare thyroid disorders, drug interactions, or laboratory-specific factors. In such cases, a thorough medical evaluation is important.
