What is Undifferentiated Arthritis?

Undifferentiated arthritis (UA) refers to an inflammatory joint condition in which, despite thorough diagnostic evaluation, it is not possible to classify the disease as a specific rheumatic condition. It often represents an early stage of inflammatory arthritis that may later evolve into a clearly defined disease such as rheumatoid arthritis.

Undifferentiated arthritis is not a final diagnosis but rather a transitional stage that requires careful monitoring and regular follow-up with a rheumatologist.

Causes

The exact causes of undifferentiated arthritis are not yet fully understood. A combination of several contributing factors is assumed:

  • Genetic predisposition: Certain gene variants, particularly within the HLA system, increase susceptibility to inflammatory joint diseases.
  • Immune dysregulation: A misdirected immune response attacks the body's own joint tissue.
  • Infections: Certain bacterial or viral infections can act as triggers.
  • Environmental factors: Smoking, stress, and other external influences may increase the risk.

Symptoms

The symptoms of undifferentiated arthritis are similar to those of other inflammatory joint diseases. Common symptoms include:

  • Pain, swelling, and warmth in one or more joints
  • Morning stiffness lasting longer than 30 minutes
  • General feeling of illness and fatigue (Fatigue)
  • Mild fever in some cases
  • Involvement of small joints (e.g., finger or toe joints) or larger joints such as the knees or wrists

Symptoms may appear intermittently or persist continuously. In some cases, they resolve spontaneously.

Diagnosis

The diagnosis of undifferentiated arthritis is established by excluding other clearly defined conditions. The rheumatological workup typically includes:

  • Medical history and physical examination: Assessment of symptoms, duration, and the distribution pattern of joint inflammation.
  • Blood tests: Measurement of inflammatory markers (CRP, ESR), rheumatoid factor (RF), anti-CCP antibodies, and antinuclear antibodies (ANA).
  • Imaging: Ultrasound and MRI of affected joints can detect early inflammatory changes or initial joint damage.
  • X-rays: To exclude structural joint damage and other conditions.

If the condition does not meet the classification criteria for rheumatoid arthritis, psoriatic arthritis, reactive arthritis, spondyloarthritis, or any other defined form, the diagnosis of undifferentiated arthritis is made.

Disease Course and Prognosis

The course of undifferentiated arthritis varies considerably between individuals:

  • In approximately 30–40% of patients, a clearly classifiable disease develops over time, most commonly rheumatoid arthritis.
  • In others, the condition remains undifferentiated or enters remission (complete resolution of symptoms).
  • A positive result for anti-CCP antibodies or rheumatoid factor is considered a risk factor for progression to rheumatoid arthritis.

Treatment

Treatment of undifferentiated arthritis is guided by the severity of symptoms and the individual risk profile of the patient. The goals are to reduce inflammation, prevent joint damage, and improve quality of life.

Pharmacological Treatment

  • Nonsteroidal anti-inflammatory drugs (NSAIDs): To relieve pain and inflammation (e.g., ibuprofen, naproxen).
  • Corticosteroids: Used short-term for rapid control of inflammation, e.g., prednisolone.
  • Disease-modifying antirheumatic drugs (DMARDs): In patients with a high risk of progression, early use of methotrexate or hydroxychloroquine may be considered to prevent the development of chronic arthritis.

Non-Pharmacological Measures

  • Physiotherapy to maintain joint function and strengthen surrounding muscles
  • Occupational therapy to adapt daily activities to the condition
  • Patient education and psychosocial support
  • Weight management in overweight patients to reduce joint load

When to See a Doctor

Persistent joint swelling, pain, or morning stiffness lasting more than 30 minutes should prompt a timely visit to a rheumatologist. Early diagnosis and treatment can significantly reduce the risk of long-term joint damage.
