Urine cell markers are biological markers detected in urine that provide information about cellular changes within the urinary tract. They are primarily used for the early detection, diagnosis, and monitoring of bladder carcinoma (bladder cancer). Unlike invasive cystoscopy (bladder endoscopy), urine cell markers offer a non-invasive method for examining the bladder.
The most well-known urine cell markers include urine cytology, the NMP22 test (Nuclear Matrix Protein 22), the BTA test (Bladder Tumor Antigen), the FISH test (Fluorescence In Situ Hybridization), and newer molecular tests such as the CxBladder test and the Xpert Bladder Cancer Monitor.
Urine cell markers are used in a variety of clinical scenarios:
Urine cytology is the classical method of microscopically examining cells shed into the urine. Cells from the lining of the urinary tract (urothelium) are analyzed for abnormal or malignant changes. The method is highly specific but less sensitive for low-grade tumors.
The NMP22 test measures Nuclear Matrix Protein 22, which is released into urine in elevated amounts during the breakdown of tumor cells. Elevated NMP22 levels may indicate the presence of a bladder carcinoma. The test is available as a rapid point-of-care screening tool.
The BTA test (Bladder Tumor Antigen test) detects a specific protein produced by bladder tumor cells. It is available as a qualitative rapid test and as a quantitative laboratory assay (BTA stat and BTA TRAK).
The FISH test uses fluorescently labeled DNA probes to identify chromosomal abnormalities in urine cells that are characteristic of bladder cancer. It is considered particularly sensitive and can also detect low-grade tumors.
Advanced tests such as CxBladder or the Xpert Bladder Cancer Monitor analyze specific gene expression patterns or mutations in urine cells and offer high diagnostic accuracy.
No single urine cell marker is sufficient on its own to definitively diagnose or rule out bladder cancer. These markers are therefore always used in combination with cystoscopy and imaging methods (e.g., ultrasound, CT urography). Interpretation of test results requires clinical expertise, as false-positive results can occur (e.g., in urinary tract infections or kidney stones).
