The more advanced technique, duplex sonography, combines real-time ultrasound imaging with flow measurement, allowing precise visualization of vein structure and blood flow. This method is painless, radiation-free, and widely available.
Photoplethysmography (PPG)
Photoplethysmography is an optical measurement technique that detects changes in blood volume within superficial veins using light reflection. A small sensor is placed on the skin and records the venous refilling time during defined movement exercises (e.g., toe raises). A shortened refilling time is an indicator of venous insufficiency.
Venous Occlusion Plethysmography
In venous occlusion plethysmography, a pressure cuff is used to temporarily interrupt venous outflow in order to measure the venous capacity and compliance of the venous system. This technique provides information about potential outflow obstructions.
Clinical Tests
In addition to instrumental examinations, simple clinical tests may also be performed, such as the Trendelenburg test (evaluating the valve function of superficial veins) and the Perthes test (checking the patency of the deep venous system).
How Is the Test Performed?
The venous circulation test is generally straightforward and comfortable for patients. Typically:
- The patient lies or sits in a relaxed position on an examination table
- Ultrasound gel or a sensor is applied to the skin

- No special preparation is required (fasting is not necessary)

Clinical Significance and Interpretation of Results
The results of the venous circulation test are crucial for diagnosis and treatment planning. Abnormal findings may indicate the following conditions:
- Chronic venous insufficiency (CVI): Weakness of the venous valves with persistent backflow of blood
- Deep vein thrombosis (DVT): Blood clots in the deep leg veins, potentially life-threatening due to the risk of pulmonary embolism
- Varicose veins: Dilated, tortuous superficial veins caused by valve failure
- Post-thrombotic syndrome: Long-term consequence of a previous thrombosis with damage to the vein wall
A normal test result largely rules out serious venous disease and provides reassurance to the patient.
Risks and Contraindications
The venous circulation test is a safe and low-risk procedure. Ultrasound examinations are non-invasive and radiation-free. There are very few contraindications; the use of an ultrasound probe may be limited in cases of open wounds or skin infections in the examination area. Allergic reactions to ultrasound gel are extremely rare.
