Things worth knowing about "Wound protection cream"

A wound protection cream shields the skin from moisture, friction, and irritants. It is used preventively to protect skin integrity and avoid wound formation.

What is a Wound Protection Cream?

A wound protection cream is a topically applied skincare product designed to protect the skin from external factors such as moisture, friction, urine, stool, and other irritants. It forms a protective barrier on the skin surface and is widely used in nursing and medical care to prevent skin lesions and pressure injuries.

Areas of Application

Wound protection creams are used in various nursing and medical contexts, including:

  • Incontinence-associated dermatitis (IAD): Protection of the perianal and perineal skin from moisture caused by urine or stool.
  • Diaper dermatitis (diaper rash): Prevention and treatment of skin irritation in the nappy area of infants and toddlers.
  • Pressure ulcer prophylaxis: Supporting skin protection in bedridden patients at pressure-prone areas.
  • Stoma care: Protecting peristomal skin from the corrosive output from the bowel or bladder.
  • Radiation dermatitis: Soothing and protecting the skin of patients undergoing radiation therapy.
  • Occupational skin protection: Use in professions with frequent contact with water, cleaning agents, or chemicals.

Active Ingredients and Composition

Wound protection creams typically contain a combination of skin-nourishing and barrier-supporting ingredients. Commonly used active ingredients include:

  • Zinc oxide: Has anti-inflammatory, astringent, and protective coating properties. A classic component of many wound protection products.
  • Dexpanthenol (Provitamin B5): Supports wound healing and promotes skin regeneration.
  • Petrolatum (Vaseline): Forms an occlusive protective layer on the skin and prevents transepidermal water loss.
  • Lanolin: A natural wool wax that keeps the skin soft and protected.
  • Dimethicone (silicone): Creates a water-repellent layer on the skin surface, shielding it from moisture.
  • Plant-based oils and extracts: Such as shea butter, calendula, or almond oil for additional care and skin soothing.

Mechanism of Action

Wound protection creams primarily work by forming a physical protective barrier on the skin. This barrier serves several functions:

  • It keeps moisture, urine, stool, and other irritants away from the skin.
  • It reduces friction between the skin and clothing or surfaces.
  • It supports the skin's natural protective function, known as the hydrolipid film.
  • Certain ingredients additionally promote wound healing and have anti-inflammatory effects.

Application and Dosage

Wound protection cream should be applied regularly and in accordance with the manufacturer's or healthcare professional's recommendations. General guidelines include:

  • The skin should be gently cleansed and patted dry before application.
  • The cream is applied in a thin, even layer to the at-risk skin area.
  • In cases of incontinence, the cream should be reapplied after each change of incontinence products.
  • For infants and toddlers, the cream is applied to the nappy area at each nappy change.

Side Effects and Tolerability

Wound protection creams are generally very well tolerated and suitable for long-term use. In rare cases, the following reactions may occur:

  • Allergic reactions: Particularly to ingredients such as lanolin, fragrances, or plant-based extracts.
  • Contact dermatitis: Skin inflammation triggered by a specific ingredient.
  • Blocked sweat or sebaceous glands: Possible with highly occlusive products during prolonged use.

If an allergy to any component of the cream is known, an alternative product should be selected. In cases of doubt, consultation with a healthcare professional is recommended.

Difference from Wound Healing Creams

It is important to distinguish wound protection creams from wound healing creams. While wound protection creams are used primarily as a preventive measure to protect intact skin from damage, wound healing creams are intended for the treatment of already existing wounds. Both types of products may contain similar ingredients and can complement each other in practice.
