A xanthine degradation product is a substance produced during the enzymatic breakdown of xanthine, a naturally occurring purine base. In the human body, xanthine is formed as an intermediate during the metabolism of adenine and guanine, two purine bases derived from the breakdown of DNA and RNA as well as from purine-rich foods such as meat, organ meats, legumes, and certain types of fish.
The degradation of xanthine takes place primarily in the liver and is catalysed by the enzyme xanthine oxidase (also known as xanthine dehydrogenase). This process occurs in two key steps:
Uric acid is therefore the most important and clinically relevant xanthine degradation product in humans. It is excreted primarily by the kidneys via urine, with a smaller fraction eliminated through the intestines.
When uric acid levels in the blood are persistently elevated -- a condition known as hyperuricaemia -- urate crystals can deposit in joints and soft tissues. This leads to the clinical condition known as gout (gouty arthritis), characterised by sudden and severe joint pain, swelling, and redness. The joint at the base of the big toe (podagra) is most commonly affected.
In rare cases of a congenital deficiency of xanthine oxidase, xanthine cannot be further broken down. This results in xanthinuria, a condition marked by elevated xanthine levels in the blood and urine. In severe cases, xanthine stones may form in the kidneys.
In addition to uric acid, the xanthine oxidase reaction also generates reactive oxygen species (free radicals). These can contribute to oxidative stress, which has been implicated in various conditions including atherosclerosis, heart failure, and inflammatory diseases.
Measurement of uric acid in the blood (serum urate) and urine is an important diagnostic parameter. Elevated levels indicate an increased risk of gout, kidney stones, or disorders of purine metabolism. Normal serum uric acid values are below 7.0 mg/dl in men and below 6.0 mg/dl in women.
