Zinc absorption markers are biochemical parameters used to measure and evaluate the uptake of zinc from food through the gastrointestinal tract. Zinc is an essential trace element that plays a central role in immune function, wound healing, cell growth, and numerous enzymatic processes. Since impaired zinc absorption can have significant health consequences, markers that reflect the body's absorptive capacity are of great clinical importance.
Zinc absorption occurs primarily in the small intestine and is influenced by several factors, including dietary composition, the presence of phytates or oxalates, overall nutritional status, and individual physiological conditions. Reduced zinc absorption can lead to zinc deficiency, which manifests as symptoms such as immune weakness, growth retardation, skin changes, and increased susceptibility to infections.
The most accurate method for determining zinc absorption is the use of stable zinc isotopes (e.g., 67Zn, 68Zn, or 70Zn). Labeled zinc compounds are administered orally or intravenously and subsequently measured in blood, urine, or stool. The ratio of absorbed isotope to the administered dose allows precise calculation of the fractional absorption of zinc (FAZ).
The serum zinc concentration is the most commonly used clinical marker for zinc status. Although it only indirectly reflects absorption, it serves as a practical screening parameter. Normal values in adults are generally between 70 and 120 µg/dl. Low serum levels may indicate impaired absorption or increased demand.
Measuring zinc excretion in 24-hour urine provides information about overall zinc status and can help differentiate between an absorption deficit and increased renal zinc loss.
