Extrauterine Pregnancy – Definition
In an extrauterine pregnancy, the fertilized egg does not implant in the lining of the uterus but instead attaches elsewhere in the body. In the majority of cases, this occurs in one of the fallopian tubes, known as a tubal pregnancy. Less commonly, implantation may occur on the ovary, in the abdominal cavity, or at the cervix.
Causes
Several risk factors can increase the likelihood of an extrauterine pregnancy:
- Previous inflammation of the fallopian tubes (e.g., caused by sexually transmitted infections such as chlamydia)
- Prior abdominal or fallopian tube surgery
- Endometriosis
- Previous ectopic pregnancy
- Use of an intrauterine device (IUD)
- Hormonal disorders affecting tubal function
Symptoms
Symptoms of an extrauterine pregnancy may initially be non-specific and resemble those of a normal early pregnancy. Common signs include:
- One-sided lower abdominal pain
- Missed menstrual period
- Light vaginal bleeding or spotting
- Dizziness, nausea, and circulatory collapse in the event of a fallopian tube rupture (medical emergency)
A ruptured fallopian tube (tubal rupture) is a life-threatening emergency requiring immediate medical intervention.
Diagnosis
The following investigations are used to diagnose an extrauterine pregnancy:
- Pregnancy test (beta-hCG blood test): A positive test without evidence of an intrauterine pregnancy raises suspicion.
- Ultrasound (sonography): Transvaginal ultrasound is used to determine the location of the pregnancy.
- Laparoscopy: In unclear cases, minimally invasive abdominal surgery may be needed to confirm the diagnosis.
Treatment
Treatment depends on the stage of the pregnancy and the overall condition of the patient:
- Medical treatment: If diagnosed early, the medication methotrexate can be used to stop the growth of the pregnancy tissue.
- Surgical treatment: In most cases, surgery is required – either laparoscopically (minimally invasive) or as open surgery. The affected fallopian tube may be preserved or removed depending on the extent of damage.
- Emergency care: In the event of a tubal rupture, immediate surgery is life-saving and essential for controlling internal bleeding.
Prognosis and Long-Term Outlook
After an extrauterine pregnancy, future pregnancies are possible in many cases. However, the risk of a subsequent ectopic pregnancy is elevated. Early diagnosis and prompt treatment significantly improve the prognosis and help preserve the patient's fertility.
