What Is Gallbladder Dyskinesia?
Gallbladder dyskinesia (also known as biliary dyskinesia) is a functional motility disorder of the gallbladder. In this condition, the gallbladder is unable to contract effectively in response to eating, resulting in impaired release of stored bile into the small intestine. Importantly, this occurs in the absence of gallstones, inflammation, or other structural abnormalities. It is classified as a functional biliary disorder and can cause significant digestive discomfort.
Causes
The exact causes of gallbladder dyskinesia are not fully understood, but several contributing factors have been identified:
- Disruption of the hormonal or neural signals that regulate gallbladder contraction, particularly the hormone cholecystokinin (CCK)
- Chronic stress and psychosomatic factors
- Hormonal changes, such as those occurring during pregnancy or with use of oral contraceptives
- Certain medications, including opioids and calcium channel blockers
- Inflammatory bowel disease or other gastrointestinal conditions
- Long-term high-fat or unbalanced diet
Symptoms
The symptoms of gallbladder dyskinesia closely resemble those of gallstones or gallbladder inflammation. Common complaints include:
- Recurring pain or pressure in the upper right abdomen, often triggered by fatty meals
- Nausea and occasional vomiting
- Bloating and a feeling of fullness
- Intolerance to fatty or fried foods
- Pain that may radiate to the right shoulder or back
Symptoms typically occur in episodes and may vary in frequency and severity.
Diagnosis
Because structural causes must first be ruled out, diagnosis involves several steps:
- Abdominal ultrasound: To exclude gallstones, polyps, or inflammation
- Hepatobiliary iminodiacetic acid (HIDA) scan: A nuclear medicine imaging test that measures gallbladder function and its ejection fraction. An ejection fraction below 35–40% is considered abnormal and indicates dyskinesia.
- Blood tests to rule out infection, liver disease, or bile duct obstruction
- Endoscopic procedures if additional differential diagnoses need to be excluded
Treatment
Conservative Management
Non-surgical approaches are usually the first line of treatment:
- Dietary modifications: Reducing fat, fried foods, and alcohol; eating smaller and more frequent meals
- Stress reduction techniques and, where appropriate, psychological support
- Medications to improve gallbladder motility (prokinetics) or relieve pain
- Review and possible adjustment of medications that impair gallbladder emptying
Surgical Treatment
For patients with persistent, severe symptoms and a confirmed diagnosis, laparoscopic cholecystectomy (minimally invasive surgical removal of the gallbladder) may be recommended. Studies indicate that many patients experience significant symptom relief after the procedure. However, the decision to operate should be made carefully on an individual basis, as not all patients benefit equally from cholecystectomy.
