What Are HIV Symptoms?
HIV (Human Immunodeficiency Virus) is a virus that attacks the immune system, specifically CD4 T-cells (also known as T-helper cells). The symptoms of an HIV infection develop in stages and can vary greatly from person to person. Many individuals do not notice any signs early on, as initial symptoms are often non-specific or entirely absent.
Stages of HIV Infection and Their Symptoms
1. Acute HIV Infection (Primary Stage)
Within a few weeks of exposure, many people develop an acute phase that resembles the flu. This is also called acute retroviral syndrome. Common symptoms include:
- Fever
- Night sweats
- Sore throat
- Swollen lymph nodes
- Skin rash (exanthem)
- Muscle and joint pain
- Headaches
- Fatigue and general weakness
- Occasional diarrhea and nausea
These symptoms typically resolve on their own within a few weeks, which is why the infection often goes undetected at this stage.
2. Latency Phase (Clinically Asymptomatic Stage)
After the acute phase, the virus enters a latency period that can last several years. During this time, HIV continues to replicate in the body, but many people experience few or no symptoms. Possible signs may include:
- Persistently swollen lymph nodes (lymphadenopathy)
- Occasional fatigue
Without treatment, this phase can last 5 to 10 years or longer.
3. Symptomatic HIV Infection
As the immune system becomes increasingly damaged, more noticeable symptoms can appear, including:
- Chronic fatigue
- Unexplained weight loss
- Recurring infections (e.g., fungal infections, herpes)
- Prolonged diarrhea
- Night sweats
- Skin changes
4. AIDS (Acquired Immunodeficiency Syndrome)
AIDS is the advanced stage of untreated HIV infection. At this point, the immune system is so severely compromised that opportunistic infections and certain cancers can occur -- conditions that are rare in healthy individuals. Examples include:
- Pneumocystis pneumonia (PCP)
- Toxoplasmosis
- Cytomegalovirus (CMV) infections
- Kaposi sarcoma
- Tuberculosis
Diagnosis
HIV is diagnosed through an HIV test that detects antibodies and/or viral antigens in the blood. The commonly used HIV combination test (ELISA) screens for both HIV-1 and HIV-2 antibodies as well as the p24 antigen. A positive result is confirmed by a follow-up test such as a Western Blot or PCR. Early testing is essential, as many people remain asymptomatic for a long time.
Treatment
While HIV cannot be cured, it can be very effectively managed with modern antiretroviral therapy (ART). ART suppresses viral replication, protects the immune system, and prevents progression to AIDS. People who consistently take ART can achieve a near-normal life expectancy, and with an undetectable viral load, the virus can no longer be transmitted to others (U=U: Undetectable = Untransmittable).
When to See a Doctor
Anyone who has had potential exposure to HIV or notices symptoms of an acute infection should see a healthcare provider promptly or get tested. Regular testing is also recommended for individuals at higher risk, even without symptoms. Many clinics, community health centers, and sexual health services offer free or confidential HIV testing.
