What is Lorazepam?

Lorazepam is a prescription medication belonging to the benzodiazepine class of drugs. It is one of the most widely prescribed anxiolytic and sedative agents worldwide. Lorazepam exerts anxiolytic (anxiety-reducing), sedative (calming), hypnotic (sleep-inducing), muscle-relaxant, and anticonvulsant (anti-seizure) effects. It is marketed under various brand names, most notably Ativan in the United States and the United Kingdom.

Indications

Lorazepam is used across a range of clinical settings:

  • Anxiety disorders and acute anxiety episodes
  • Insomnia associated with significant psychological distress
  • Epileptic seizures and status epilepticus (emergency treatment)
  • Pre-operative sedation and premedication
  • Alcohol withdrawal syndrome
  • Agitation and restlessness in intensive care settings

Mechanism of Action

Lorazepam acts by binding to the GABA-A receptor, the primary inhibitory neurotransmitter receptor in the central nervous system. By binding to a specific site on this receptor, lorazepam enhances the effect of gamma-aminobutyric acid (GABA), thereby reducing neuronal excitability. This results in the characteristic calming, anti-anxiety, and anticonvulsant effects of the drug.

Dosage and Administration

The appropriate dosage of lorazepam depends on the indication, the age of the patient, and individual response. It is available in several formulations:

  • Oral tablets (typically 0.5 mg, 1 mg, 2 mg)
  • Injectable solution for intravenous or intramuscular use (e.g., in emergency situations)
  • Sublingual tablets for faster absorption under the tongue

For adults with anxiety disorders, the typical daily dose ranges from 1 mg to 4 mg, divided into multiple doses. Lorazepam is a prescription-only medication and should never be used without medical supervision.

Side Effects

As with all benzodiazepines, lorazepam can cause a number of side effects:

  • Drowsiness and sedation (most common)
  • Dizziness and impaired coordination
  • Memory impairment and difficulty concentrating
  • Muscle relaxation with an increased risk of falls (particularly in elderly patients)
  • Risk of dependence and tolerance with prolonged use
  • Paradoxical reactions (e.g., agitation, aggression) in rare cases
  • Respiratory depression at high doses or when combined with other CNS depressants

Contraindications and Precautions

Lorazepam should not be used or should be used only with strict medical supervision in patients with:

  • Known hypersensitivity to benzodiazepines
  • Myasthenia gravis (a neuromuscular disorder)
  • Severe hepatic (liver) impairment
  • Sleep apnea syndrome
  • Pregnancy and breastfeeding
  • A history of alcohol or substance dependence

Due to its dependence potential, lorazepam should only be used short-term -- generally no longer than 2 to 4 weeks -- and under close medical supervision. Abrupt discontinuation can cause withdrawal symptoms; dosage should be tapered gradually.

Drug Interactions

Lorazepam may interact with a number of other substances:

  • Alcohol: Enhanced CNS depression and increased risk of respiratory depression
  • Other benzodiazepines and sedatives: Additive sedative effects
  • Opioids: Significantly increased risk of severe respiratory depression and death
  • Antidepressants and antipsychotics: Mutual potentiation of CNS effects
  • Clozapine: Risk of cardiac arrest and respiratory depression
