What is Actigraphy?

Actigraphy is a non-invasive, objective diagnostic technique used to continuously record physical movement and rest patterns over extended periods of time. A small, wrist-worn device called an actigraph uses an integrated accelerometer to detect and store motion data over days or even weeks. This data is then used to draw conclusions about an individual´s sleep-wake cycle, circadian rhythm, and overall physical activity patterns.

How Does Actigraphy Work?

The actigraph contains a miniaturized motion sensor (accelerometer) that continuously registers even the smallest wrist movements and saves them digitally. Periods of high movement are interpreted as wakefulness, while periods of low or no movement are classified as sleep. Many modern devices also include a light sensor to measure ambient light exposure, providing additional information relevant to the circadian rhythm.

Procedure

  • The patient wears the device continuously on the wrist, typically for 7 to 14 days.
  • A sleep diary is often kept in parallel to compare subjective perceptions with objective measurements.
  • After the recording period, data is downloaded and analyzed using specialized software.
  • Key parameters derived include sleep onset latency, total sleep time, sleep efficiency, and wake after sleep onset.

Clinical Applications

Actigraphy is widely used in sleep medicine, neurology, psychiatry, and chronobiology. Common indications include:

  • Sleep disorders such as insomnia, hypersomnia, and circadian rhythm sleep-wake disorders
  • Circadian rhythm disturbances (e.g., shift work disorder, jet lag disorder, delayed sleep phase syndrome)
  • Assessment of sleep quality and quantity in a real-life home environment
  • Monitoring and follow-up in depression and bipolar disorder
  • Evaluation of activity patterns in ADHD and related conditions
  • Supporting diagnosis of restless legs syndrome and periodic limb movements during sleep

Advantages of Actigraphy

Compared to polysomnography – the gold standard in sleep diagnostics – actigraphy offers several practical benefits:

  • Measurements are taken in the patient´s natural home sleep environment
  • Cost-effective and easy to use
  • Enables long-term recording over multiple days or weeks
  • Avoids the so-called first-night effect associated with laboratory-based sleep studies

Limitations

Actigraphy also has limitations: it cannot directly measure sleep stages such as deep sleep or REM sleep, and it may be less reliable in patients with reduced motor activity (e.g., Parkinson's disease) or restricted arm movement. Additionally, it may not always clearly distinguish between sleep and quiet wakefulness. For detailed sleep stage analysis, polysomnography in a sleep laboratory remains necessary.
