What are ADHD symptoms?

ADHD stands for Attention Deficit Hyperactivity Disorder and is one of the most common neuropsychiatric conditions in children and adolescents, often persisting into adulthood. Symptoms are grouped into three core categories: inattention, hyperactivity, and impulsivity. Depending on how these categories are expressed, different presentations of ADHD are recognized.

Core Symptoms of ADHD

Inattention

  • Difficulty sustaining attention during tasks or play activities
  • Frequently forgetting daily tasks or appointments
  • Easily distracted by external stimuli
  • Careless mistakes due to lack of attention to detail
  • Losing important items such as keys or notebooks
  • Difficulty organizing tasks and activities

Hyperactivity

  • Persistent physical restlessness, fidgeting, or squirming in seat
  • Frequently leaving seat in situations where remaining seated is expected
  • Excessive running or climbing in inappropriate situations
  • Difficulty engaging in leisure activities quietly
  • Often appearing to be driven by a motor

Impulsivity

  • Blurting out answers before questions have been completed
  • Difficulty waiting for one's turn

ADHD Symptoms in Children vs. Adults

In children, hyperactivity and impulsivity are often the most prominent features. In adults, physical restlessness tends to diminish, while inner restlessness, concentration difficulties, forgetfulness, and problems with work organization become more apparent. Many adults are only diagnosed later in life because their symptoms went unrecognized for years.

ADHD Presentations

  • Predominantly inattentive presentation (formerly ADD): The main feature is inattention without marked hyperactivity. More common in girls and women.
  • Predominantly hyperactive-impulsive presentation: The main features are hyperactivity and impulsivity without marked inattention.
  • Combined presentation: Both inattention and hyperactivity-impulsivity are significantly present. This is the most common type in children.

Associated Symptoms and Comorbidities

ADHD frequently co-occurs with other mental or neurological conditions, known as comorbidities:

  • Learning difficulties (e.g., dyslexia, dyscalculia)
  • Anxiety disorders
  • Depression
  • Oppositional defiant disorder or conduct disorder
  • Sleep disturbances
  • Emotional dysregulation (intense, difficult-to-control emotional reactions)

When to See a Doctor

If the symptoms listed above persist for at least six months, appear in more than one setting – such as at home and at school or work – and significantly interfere with daily functioning, it is advisable to consult a physician or psychologist. Early diagnosis and treatment substantially improves quality of life.

Diagnosis

ADHD is diagnosed by specialists in psychiatry, child and adolescent psychiatry, or neurology. The diagnosis is based on a thorough medical history, standardized questionnaires (e.g., based on DSM-5 or ICD-10/ICD-11 criteria), behavioral observation, and the exclusion of other conditions.

Treatment

ADHD is highly treatable. A multimodal approach is usually recommended:

  • Behavioral therapy: Particularly effective in children; strategies for self-regulation and daily structure.
  • Medication: Stimulants such as methylphenidate (e.g., Ritalin) or amphetamines, as well as non-stimulants such as atomoxetine, are well-supported by clinical evidence.
  • Parent training and psychoeducation: Education and support for individuals with ADHD and their families.
  • Occupational therapy and learning support: Assistance with academic and everyday challenges.
