What is Aortic Valve Replacement?

Aortic valve replacement (AVR) is a cardiac surgical procedure in which a damaged or diseased aortic valve is replaced with a prosthetic valve. The aortic valve is located between the left ventricle of the heart and the aorta (the main artery of the body) and ensures that blood flows in the correct direction. When the valve is no longer functioning properly due to disease or wear, replacement may become necessary.

Causes and Indications

Aortic valve replacement is recommended when the aortic valve is severely diseased and other treatment approaches are insufficient. The most common reasons for aortic valve disease include:

  • Aortic stenosis: The valve is narrowed and restricts blood flow. Age-related calcification is a frequent cause.
  • Aortic regurgitation (insufficiency): The valve does not close completely, causing blood to leak back into the left ventricle.
  • Congenital heart defects: For example, a bicuspid aortic valve (a valve with two leaflets instead of three).
  • Rheumatic fever: An inflammatory condition that can damage the heart valves.
  • Infective endocarditis: A bacterial infection of the heart valves.

Types of Aortic Valve Replacement

Mechanical Valve Prostheses

Mechanical valves are made from durable materials such as titanium or carbon. They are highly long-lasting and generally do not need to be replaced. However, patients with a mechanical valve must take anticoagulant medications (blood thinners) for life to prevent blood clots from forming on the valve.

Biological Valve Prostheses (Bioprostheses)

Biological valves are made from animal tissue (usually porcine or bovine) or from human donor tissue. In most cases, they do not require long-term anticoagulation therapy. However, their lifespan is limited to approximately 10 to 20 years, after which a second procedure may be required.

Minimally Invasive Aortic Valve Replacement (TAVI)

Transcatheter Aortic Valve Implantation (TAVI), also known as TAVR (Transcatheter Aortic Valve Replacement), is a minimally invasive procedure in which a new valve is delivered via a catheter -- most commonly through the femoral artery in the groin -- without the need to open the chest. This approach is particularly suitable for elderly patients or those at high surgical risk.

Procedure

In conventional (open) aortic valve replacement, the patient is placed under general anesthesia. The surgeon opens the chest through an incision in the breastbone (sternotomy) and connects the patient to a heart-lung machine, which maintains circulation and oxygenation during the procedure. The diseased valve is removed and the new valve is sutured into place. The heart is then restarted and the chest is closed.

Risks and Complications

As with any major surgical procedure, aortic valve replacement carries certain risks. Possible complications include:

  • Bleeding during or after surgery
  • Infections, including wound infections or endocarditis
  • Stroke or other neurological complications
  • Heart rhythm disorders (arrhythmias)
  • Kidney problems
  • Valve prosthesis dysfunction

The individual risk depends on the overall health of the patient, their age, and any pre-existing medical conditions.

Aftercare and Rehabilitation

Following the procedure, patients typically undergo cardiac rehabilitation, which includes supervised physical exercise, nutritional counseling, and psychological support. Regular follow-up appointments with a cardiologist are necessary to monitor the function of the valve prosthesis. Patients with mechanical valves must take anticoagulants (e.g., warfarin or newer agents) indefinitely and have their effectiveness monitored regularly through blood tests.

Prognosis

Aortic valve replacement is a well-established procedure with excellent long-term outcomes. Most patients experience a significant improvement in quality of life after the procedure, with reduction of symptoms such as shortness of breath and chest pain, as well as improved life expectancy. The choice of valve type and surgical approach is tailored individually to each patient.
