What is the Cabrera Circle?
The Cabrera circle is a diagnostic tool used in electrocardiography (ECG) that arranges the six limb leads in a circular, anatomically logical sequence. It was named after Venezuelan cardiologist Enrique Cabrera and significantly improved the systematic analysis of the electrocardiogram. Unlike the traditional ECG layout, the Cabrera circle arranges leads in a continuous sequence from −150° to +210°, making it much easier to visually identify conduction abnormalities and myocardial infarctions.
Structure and Layout
The Cabrera circle organizes the six standard limb leads (I, II, III, aVR, aVL, aVF) in a clockwise, consecutive arrangement. The order is: aVL, I, −aVR, II, aVF, III. This layout forms a 360-degree circle, with each lead separated by 30 degrees from the next.

- aVF: +90°
- III: +120°
A key distinction from conventional ECG presentation is the use of −aVR (inverted aVR) instead of the standard aVR lead. This inversion creates a seamless, anatomically meaningful sequence of leads.
Clinical Significance
Cardiac Axis Determination
The Cabrera circle allows clinicians to quickly and intuitively determine the electrical cardiac axis, which describes the dominant direction of electrical activation spreading through the heart. Deviations in the cardiac axis may indicate various conditions:
- Left axis deviation: Suggestive of left bundle branch block, left anterior hemiblock, or obesity
- Right axis deviation: Associated with right bundle branch block, right ventricular hypertrophy, or pulmonary embolism
- Extreme right axis deviation: May indicate ventricular tachycardia or severe right heart strain
Myocardial Infarction Diagnosis
The Cabrera circle greatly simplifies the localization of myocardial infarctions. Because the leads are arranged in anatomical sequence, contiguous ST-segment elevations or pathological Q-waves can be directly read as indicators of infarction in a specific coronary artery territory. Anterior, inferior, and lateral infarctions can thus be systematically identified.
Conduction Disorders
The circle also improves the visualization of bundle branch blocks and hemiblocks. The consecutive lead arrangement makes it easier to recognize patterns of delayed electrical conduction within a logical, uninterrupted flow of leads.
