What is Cannabidiol?
Cannabidiol (commonly abbreviated as CBD) is a naturally occurring chemical compound found in the hemp plant (Cannabis sativa). Unlike tetrahydrocannabinol (THC) – the most well-known psychoactive compound in cannabis – CBD does not produce a "high" or intoxicating effect. CBD belongs to a class of compounds known as cannabinoids, of which more than 100 have been identified to date.
Mechanism of Action
CBD exerts its effects primarily through the body's endocannabinoid system, a complex regulatory network consisting of receptors (CB1 and CB2) distributed throughout various tissues and organs. This system plays a key role in regulating pain perception, mood, sleep, immune responses, and inflammatory processes.
Unlike THC, CBD binds only weakly to CB1 and CB2 receptors but modulates them indirectly through multiple pathways. It also interacts with other receptor systems, including the serotonin receptor (5-HT1A), which may contribute to its reported anxiolytic (anti-anxiety) properties.
Medical Applications
Approved Pharmaceutical Use
In clinical medicine, CBD is the active ingredient in the prescription drug Epidiolex (approved in the US and EU). It is indicated for the treatment of rare, severe forms of epilepsy, including:
- Dravet syndrome
- Lennox-Gastaut syndrome
- Tuberous sclerosis complex
The efficacy of CBD for these conditions has been demonstrated in clinical trials and is recognized by regulatory bodies including the FDA (U.S. Food and Drug Administration) and the EMA (European Medicines Agency).
Areas of Ongoing Research
Current scientific research is investigating the potential use of CBD for:
- Anxiety disorders and depression
- Chronic pain and inflammation
- Sleep disorders
- Neurodegeneration (e.g., Parkinson's disease, Alzheimer's disease)
- Addiction and withdrawal symptoms
For most of these applications, large-scale clinical trials are still lacking, and official approvals have not yet been granted.
CBD as a Dietary Supplement
CBD is widely available in many countries as a dietary supplement in the form of oils, capsules, tinctures, and topical creams. The legal status varies significantly by country. In the European Union, CBD is classified as a novel food, meaning products must obtain regulatory authorization before being marketed as food items.
It is important to note that dietary supplement products containing CBD are not subject to the same quality and efficacy standards as approved pharmaceutical drugs.
Dosage
The optimal dose of CBD depends on the intended use, body weight, and individual tolerance. In a medical context, dosing is always determined by a physician. No universally accepted dosage guidelines exist for dietary supplements. The European Food Safety Authority (EFSA) has provisionally assessed a daily intake of up to 70 mg as safe for most adults.
Side Effects and Safety
CBD is generally considered well-tolerated. Possible side effects include:
- Fatigue and drowsiness
- Changes in appetite
- Diarrhea and gastrointestinal discomfort
- Elevated liver enzyme levels (at high doses)
Importantly, CBD can interact with other medications because it is metabolized by liver enzymes (CYP450 pathway). Individuals taking blood thinners, antiepileptic drugs, or other medications should consult a healthcare professional before using CBD products.
