Croup (medically known as stenosing laryngotracheitis or pseudocroup) is an inflammatory condition affecting the larynx (voice box) and trachea (windpipe), primarily occurring in children aged six months to six years. The term distinguishes the condition from true croup caused by diphtheria, which has largely been eliminated through vaccination. Croup is most common in autumn and winter and often causes sudden, frightening symptoms at night.
Croup is almost always caused by viral infections. The most common pathogens include:
These viruses inflame and swell the mucous membranes of the larynx and trachea, narrowing the airway and making breathing more difficult. In some children, allergic reactions or exposure to cold air can also trigger episodes.
Symptoms of croup typically appear suddenly, often in the middle of the night, and include:
Symptoms tend to be milder during the day and worsen at night. Seek immediate medical attention if the child shows signs of severe breathing difficulty, bluish discoloration of the lips (cyanosis), or extreme agitation.
Croup is classified into four severity grades:
Diagnosis is usually made clinically, based on the characteristic symptoms and physical examination. The barking cough and inspiratory stridor are typically sufficient for a diagnosis. In more severe or uncertain cases, the following assessments may be performed:
Keeping the child calm is essential, as agitation and crying can worsen breathing difficulty. Cool, moist air – such as opening a window or going outside briefly – may provide some relief. Humidified air is often recommended, although scientific evidence for its benefit remains inconclusive.
Children with grade 3 or 4 croup, suspected bacterial superinfection, or those under six months of age should be admitted to hospital for close monitoring and, if necessary, intensive care support.
Croup is in most cases a self-limiting illness that resolves without complications. Most children recover fully within three to seven days. Some children experience recurrent episodes during the toddler years, but as the airways grow larger with age, croup tends to become less frequent and eventually stops occurring altogether.
