Analyzing a cytokine profile provides valuable information about a person´s current immune status and activity.

Function and Importance of Cytokines

Cytokines act as messenger molecules of the immune system. The most important groups include:

  • Interleukins (IL): e.g., IL-1, IL-6, IL-10, IL-17 – regulate inflammation and immune cell communication
  • Tumor Necrosis Factors (TNF): e.g., TNF-alpha – promote inflammatory responses and cell death
  • Interferons (IFN): e.g., IFN-gamma – essential in defense against viruses and tumor cells
  • Chemokines: guide the migration of immune cells to sites of inflammation
  • Growth Factors: e.g., TGF-beta – regulate cell growth and tissue repair

Diagnostics: How Is a Cytokine Profile Measured?

Several laboratory methods are available to analyze cytokine profiles:

Multiplex Immunoassays

Modern multiplex assays (e.g., Luminex technology) allow up to 30 or more cytokines to be measured simultaneously from a single blood sample, providing a comprehensive picture of immune activity.

ELISA (Enzyme-Linked Immunosorbent Assay)

ELISA is a well-established technique for the quantitative measurement of individual cytokines. It is precise and widely used, though it measures only one cytokine per test run.

Flow Cytometry

Flow cytometry enables cytokine measurement at the single-cell level and additionally provides information about which immune cell populations are producing specific cytokines.

Transcriptomic Methods

Using RNA-based techniques (e.g., qPCR, RNA sequencing), the gene expression of cytokine-encoding genes can be analyzed, offering insights into the activity levels of immune cells.

Interpreting the Cytokine Profile

Interpreting a cytokine profile requires medical expertise, as cytokine levels depend on many factors including time of day, age, sex, comorbidities, and acute infections. Key interpretation aspects include:

  • Pro-inflammatory cytokines (e.g., IL-1beta, IL-6, TNF-alpha, IL-17): elevated in active inflammation, infections, or autoimmune conditions
  • Anti-inflammatory cytokines (e.g., IL-10, TGF-beta): regulate and dampen immune responses; imbalance may contribute to chronic inflammation

  • IL-6 inhibitors (e.g., tocilizumab): used in severe COVID-19 and rheumatoid arthritis

  • IL-4/IL-13 inhibitors (e.g., dupilumab): used in atopic dermatitis and asthma
