What is Empagliflozin?

Empagliflozin is a prescription medication belonging to the class of SGLT-2 inhibitors (sodium-glucose cotransporter-2 inhibitors). It is approved for the treatment of type 2 diabetes mellitus, heart failure, and chronic kidney disease (CKD). It is marketed under the brand name Jardiance.

Mechanism of Action

Empagliflozin works by blocking the SGLT-2 transporter in the kidney tubules. This transporter is normally responsible for reabsorbing glucose from the urine back into the bloodstream. By inhibiting this transporter, excess glucose is excreted through the urine, lowering blood sugar levels without requiring insulin.

  • Inhibition of renal glucose reabsorption
  • Increased excretion of glucose and sodium via urine (glucosuria and natriuresis)
  • Blood pressure reduction through osmotic diuresis
  • Reduced cardiac preload and afterload, supporting heart function
  • Nephroprotective effects by lowering intraglomerular pressure

Indications

Type 2 Diabetes Mellitus

Empagliflozin is used as monotherapy or in combination with other antidiabetic agents such as metformin or insulin to improve blood glucose control in adults with type 2 diabetes when lifestyle changes and other medications are insufficient.

Heart Failure

Empagliflozin is approved for heart failure with both reduced and preserved ejection fraction (HFrEF and HFpEF). Landmark trials such as EMPEROR-Reduced and EMPEROR-Preserved demonstrated significant reductions in cardiovascular death and hospitalizations for heart failure.

Chronic Kidney Disease

Empagliflozin slows the progression of chronic kidney disease and reduces the risk of kidney failure requiring dialysis. The EMPA-KIDNEY trial confirmed these nephroprotective benefits even in patients without diabetes.

Dosage

The standard dose for adults is 10 mg once daily, taken orally with or without food. The dose may be increased to 25 mg once daily for additional blood glucose control. Regular monitoring of kidney function (eGFR) is recommended. Dosing adjustments should always be made in consultation with a healthcare professional.

Side Effects

Common Side Effects

  • Genital yeast infections (Candida), particularly in women
  • Urinary tract infections
  • Increased urination (polyuria)
  • Thirst and increased fluid intake

Rare but Serious Side Effects

  • Ketoacidosis: A rare but life-threatening condition with elevated ketone levels in the blood, which can occur even at normal blood glucose levels (euglycemic diabetic ketoacidosis)
  • Fournier gangrene: A very rare but severe necrotizing fasciitis of the genital area
  • Low blood pressure (hypotension), especially when combined with diuretics
  • Acute kidney injury, particularly in cases of dehydration

Contraindications and Precautions

Empagliflozin should not be used in the following situations:

  • Type 1 diabetes (not approved for this indication)
  • Severe kidney impairment (eGFR below 20 ml/min/1.73 m², depending on indication)
  • Hypersensitivity to the active substance
  • Pregnancy and breastfeeding

Empagliflozin should be temporarily discontinued before surgical procedures or during prolonged fasting to reduce the risk of ketoacidosis.
