What Is Glutathione Peroxidase Analysis?

Glutathione peroxidase analysis is a diagnostic test that measures the activity of the enzyme glutathione peroxidase (GPx) in blood or other body fluids. Glutathione peroxidases are a family of enzymes that play a central role in the body's antioxidant defense system. They protect cells from oxidative damage by neutralizing reactive oxygen species, commonly known as free radicals.

Because most glutathione peroxidase enzymes contain the trace element selenium as an essential cofactor, measuring GPx activity is also used as an indirect marker for the body's selenium status. The analysis is primarily applied in laboratory diagnostics to evaluate oxidative stress, nutritional deficiencies, and various chronic diseases.

Background: The Glutathione Peroxidase Enzyme

Glutathione peroxidases (GPx1 through GPx8) are found in various tissues, including red blood cells (erythrocytes), the liver, kidneys, and blood plasma. Their primary function is to convert hydrogen peroxide and lipid hydroperoxides into harmless compounds, thereby preventing cellular damage. Most GPx enzymes require the selenium-containing amino acid selenocysteine at their active site to perform this catalytic function.

When Is Glutathione Peroxidase Analysis Indicated?

This test is used in a variety of clinical and research settings. Common indications include:

  • Suspected selenium deficiency or selenium excess
  • Evaluation of elevated oxidative stress, for example in chronic inflammatory conditions
  • Monitoring of nutritional therapy and supplementation programs
  • Assessment of the antioxidant defense system in patients with cancer or cardiovascular disease
  • Research into aging processes and degenerative diseases
  • Follow-up in patients with thyroid conditions such as Hashimoto thyroiditis, as the thyroid gland has a particularly high demand for selenium

How Is the Analysis Performed?

The glutathione peroxidase analysis typically requires a venous blood sample. Enzyme activity can be measured in whole blood, erythrocytes, or blood plasma. The erythrocyte GPx activity is generally considered the most informative, as it reflects the long-term selenium status of the body.

In the laboratory, GPx activity is most commonly determined using a spectrophotometric method: the enzyme activity is measured by tracking the oxidation of reduced glutathione (GSH) to oxidized glutathione (GSSG) in the presence of a substrate. Results are typically expressed in units per gram of hemoglobin (U/g Hb) or units per liter (U/L).

Interpreting the Results

Reference ranges may vary depending on the laboratory, measurement method, and sample type. In general:

  • Low GPx activity: May indicate selenium deficiency, elevated oxidative stress, or increased consumption of the antioxidant system. It is also observed in conditions such as renal insufficiency, liver cirrhosis, or malnutrition.
  • Elevated GPx activity: May reflect a compensatory response to chronic oxidative stress, for example in inflammatory diseases or intense physical training.

Results should always be interpreted in the context of other laboratory values, particularly the serum selenium level, inflammatory markers (such as CRP), and other antioxidant parameters.

Clinical Significance and Related Conditions

A well-functioning antioxidant system is essential for overall health. Reduced GPx activity has been associated with a wide range of conditions, including:

  • Cardiovascular diseases (e.g., atherosclerosis)
  • Chronic inflammatory conditions (e.g., rheumatoid arthritis)
  • Neurodegenerative diseases (e.g., Parkinson's disease, Alzheimer's disease)
  • Diabetes mellitus and its complications
  • Certain types of cancer

The glutathione peroxidase analysis is therefore a valuable tool in preventive and functional medicine, enabling early identification of health risks and guiding targeted therapeutic measures such as selenium supplementation or antioxidant-focused dietary adjustments.
