What is Iron Oxide?

Iron oxide refers to a group of chemical compounds composed of iron and oxygen. The most common forms include iron(II) oxide (FeO), iron(III) oxide (Fe2O3, known as hematite or rust), and iron(II,III) oxide (Fe3O4, magnetite). In medicine, pharmacy, and the food industry, iron oxides play an important role as colorants, contrast agents, and carrier systems for active pharmaceutical ingredients.

Occurrence and Production

Iron oxides occur naturally in ores, soils, and minerals. For medical and industrial use, they are produced synthetically to ensure defined purity, particle size, and stability. Synthetic iron oxides are often manufactured as nanoparticles, which exhibit distinctive physical and chemical properties relevant to biomedical applications.

Medical Applications

Colorants in Pharmaceuticals and Food

Iron oxides are approved colorants used in pharmaceuticals and the food industry. In the European Union, they are classified under E172 (iron oxides and hydroxides) as permitted food colorants. In pharmaceutical products, they are used to color tablets, capsules, and film-coated tablets, allowing for easy product identification and aesthetic presentation.

Contrast Agents in Medical Imaging

Superparamagnetic iron oxide nanoparticles (SPIONs) are used as MRI contrast agents. They accumulate preferentially in specific tissues such as the liver and lymph nodes, altering the MRI signal and enabling improved visualization of tumors, metastases, and inflammatory lesions. Compared to gadolinium-based contrast agents, iron oxide nanoparticles are generally considered to have a favorable tolerability profile.

Iron Replacement Therapy

Certain iron oxide compounds are used for intravenous treatment of iron deficiency, particularly when oral iron therapy is not feasible or sufficiently effective. Preparations such as ferumoxytol are based on iron oxide nanoparticles and release iron in a controlled manner to support hemoglobin production in the body.

Drug Delivery Systems

In modern nanomedicine, iron oxide nanoparticles are being investigated as drug carrier platforms. Their magnetic properties allow them to be guided to specific locations in the body using external magnetic fields, which may improve targeted drug delivery, particularly in cancer therapy.

Safety and Tolerability

Iron oxides are generally considered safe when used as intended. As food colorants (E172), they are poorly absorbed by the body and are regarded as safe at typical usage levels. With intravenous administration, allergic or infusion-related reactions may rarely occur. The biological effects of iron oxide nanoparticles can vary depending on their size and surface properties; long-term safety continues to be studied.

Interactions

When using intravenous iron oxide preparations, potential interactions with other medications should be considered. Tetracycline antibiotics and certain bisphosphonates taken concurrently may influence iron absorption. Patients should inform their doctor or pharmacist about all medications they are taking.
