Liver Stiffness: Causes, Measurement & Treatment

Liver stiffness describes the hardening of liver tissue caused by scarring (fibrosis). It is measured to assess the severity of liver disease non-invasively.

What is Liver Stiffness?

Liver stiffness (also referred to as hepatic stiffness or liver elasticity) describes the degree of hardening of the liver tissue. A healthy liver is relatively soft and elastic. When the liver is damaged – for example by chronic inflammation, excessive alcohol consumption, or metabolic disorders – it forms scar tissue, a process known as fibrosis. This scar tissue increases the measurable stiffness of the organ. Measuring liver stiffness is an important diagnostic tool for assessing the severity of liver fibrosis or cirrhosis without the need for a tissue biopsy.

Causes of Increased Liver Stiffness

Increased liver stiffness results from chronic damage to the liver tissue. The most common causes include:

  • Chronic Hepatitis B and C: Viral liver inflammation that can lead to fibrosis over years.
  • Alcoholic Liver Disease: Long-term excessive alcohol consumption damages liver cells and promotes scarring.
  • Non-Alcoholic Fatty Liver Disease (NAFLD/MASLD): Fat deposits in the liver, often associated with obesity, diabetes, and metabolic syndrome.
  • Autoimmune Hepatitis: An overactive immune response targets the liver tissue.
  • Primary Biliary Cholangitis (PBC) and Primary Sclerosing Cholangitis (PSC): Bile duct diseases that lead to liver fibrosis.
  • Haemochromatosis and other storage diseases: Deposits of iron or other substances in the liver tissue.

Diagnosis: Measuring Liver Stiffness

Liver stiffness is today primarily measured using non-invasive methods that can replace a liver biopsy in many cases.

Transient Elastography (FibroScan)

Transient elastography, widely known by the brand name FibroScan, is the most commonly used method. An ultrasound probe sends a sound wave into the liver tissue. The speed at which this wave propagates is directly proportional to the hardness of the tissue. Stiffness is expressed in kilopascals (kPa). Normal values are approximately 2–7 kPa. Higher values indicate advanced fibrosis or cirrhosis.

Magnetic Resonance Elastography (MRE)

Magnetic resonance elastography combines MRI technology with elastography principles. It provides a highly accurate, wide-area map of liver stiffness and is considered particularly precise, though it is more complex and costly than FibroScan.

Acoustic Radiation Force Impulse Elastography (ARFI)

ARFI elastography is an ultrasound-based method that can be integrated directly into modern ultrasound devices, allowing targeted measurement in a selected region of the liver.

Blood Tests (Indirect Markers)

In addition to imaging-based elastography, blood tests can provide indirect indications of the degree of fibrosis – for example, the FIB-4 score (based on age, liver enzyme levels, and platelet count) or the APRI score.

Interpretation of Measured Values

The classification of stiffness values depends on the underlying condition. As a general guide for transient elastography:

  • Below 7 kPa: Generally no significant finding (F0–F1, no or minimal fibrosis).
  • 7–9.5 kPa: Significant fibrosis possible (F2).
  • 9.5–12.5 kPa: Advanced fibrosis likely (F3).
  • Above 12.5 kPa: Liver cirrhosis likely (F4).

These threshold values can vary depending on the underlying disease and clinical guidelines. Elevated liver stiffness should always be evaluated in its clinical context by a qualified physician.

Treatment and Prognosis

Liver stiffness itself is not a disease but a diagnostic parameter. Treatment is directed at the underlying condition:

  • Antiviral therapy for Hepatitis B or C can significantly reverse fibrosis and reduce liver stiffness.
  • Alcohol abstinence leads to a measurable improvement in stiffness in alcoholic liver disease.
  • Weight loss and lifestyle changes are key for NAFLD/MASLD to reduce fatty liver and associated fibrosis.
  • Drug therapies for liver fibrosis are being intensively researched; some newer agents (e.g., for MASH) have shown promising results in clinical trials.

Regular follow-up measurements of liver stiffness allow objective monitoring of treatment success. Early diagnosis and treatment are essential to prevent liver cirrhosis and its serious complications.
