What is Malaria tropica?

Malaria tropica is the most dangerous and most frequently fatal form of malaria. It is caused by the single-celled parasite Plasmodium falciparum and is primarily transmitted through the bite of an infected female Anopheles mosquito. Malaria tropica occurs mainly in tropical and subtropical regions, particularly in sub-Saharan Africa, South Asia, and parts of South America. Compared to other forms of malaria, it progresses much more rapidly and can be fatal within days if not treated promptly.

Causes and Transmission

The disease is triggered by the parasite Plasmodium falciparum, which enters the human bloodstream through the bite of an infected Anopheles mosquito. The parasites first travel to the liver, where they multiply, before invading and destroying red blood cells. Transmission between humans via the bloodstream -- for example through blood transfusions or shared needles -- is also possible, but significantly less common.

Symptoms

The incubation period is typically 7 to 14 days after the mosquito bite. Common symptoms include:

High fever, often without the periodic fever pattern seen in other forms of malaria
Chills and heavy sweating
Severe headaches
Muscle and joint pain
Nausea, vomiting, and diarrhea
Extreme fatigue and weakness

Without timely treatment, malaria tropica can lead to severe complications, including:

Cerebral malaria (involvement of the brain, leading to impaired consciousness, seizures, and coma)
Severe anemia due to destruction of red blood cells
Kidney failure
Respiratory failure (ARDS)
Multi-organ failure and death

Diagnosis

Diagnosis is made quickly and accurately using the following methods:

Thick blood smear microscopy: The gold standard, in which blood is examined under a microscope for parasites.
Rapid diagnostic tests (RDT): Antigen-based tests that detect Plasmodium falciparum antigens in the blood and provide results within minutes.
PCR (polymerase chain reaction): A highly sensitive molecular method for detecting the pathogen, especially at low parasite densities.

Early diagnosis is critical, as malaria tropica can deteriorate rapidly.

Treatment

Malaria tropica is a medical emergency and must be treated immediately. Therapy depends on the severity of the illness and regional resistance patterns:

Uncomplicated Malaria tropica

Artemether/lumefantrine (e.g., Riamet®): First-line treatment in many countries
Atovaquone/proguanil (e.g., Malarone®): A well-tolerated alternative
Dihydroartemisinin/piperaquine: Another available option

Severe Malaria tropica

Intravenous artesunate: Treatment of choice for severe cases, largely replacing the previously used quinine
Intensive care management for complications such as cerebral malaria or organ failure

Prevention

Prophylaxis against malaria tropica is essential for travelers to endemic regions:

Chemoprophylaxis: Atovaquone/proguanil, mefloquine, or doxycycline depending on the destination and individual factors
Exposure prevention: Use of insect repellents, impregnated mosquito nets, and protective clothing, especially at dusk and during the night
Vaccination: The malaria vaccine RTS,S/AS01 (Mosquirix) has been recommended by the WHO since 2021 for children in sub-Saharan Africa
