What is Minoxidil?

Minoxidil is a pharmaceutical compound originally developed as a medication for high blood pressure (hypertension). During clinical use, it was discovered that minoxidil promotes hair growth as a side effect. Today, it is primarily used to treat androgenetic alopecia (hereditary hair loss) in both men and women. It is available as a topical solution, foam, and oral tablet.

Mechanism of Action

The exact mechanism by which minoxidil promotes hair growth is not yet fully understood. It is known that minoxidil acts as a potassium channel opener, causing vasodilation (widening of blood vessels) in the scalp. This improves blood flow to hair follicles and enhances their supply of oxygen and nutrients. Additionally, minoxidil prolongs the anagen phase (growth phase) of the hair follicle and shortens the telogen phase (resting phase), resulting in increased hair density and length.

Indications

  • Androgenetic alopecia in men and women
  • Alopecia areata (patchy hair loss) – as adjunctive therapy
  • Hypertension – high-dose oral form used in combination with other antihypertensives

Dosage and Application

Topical Application

The most common form for treating hair loss is topical application directly to the scalp. For men, a 5% solution or foam is typically recommended, while women may use a 2% or 5% solution. Application is generally performed twice daily. First visible results usually appear after 3 to 6 months of consistent use.

Oral Application

At low doses (e.g., 0.25 to 5 mg daily), oral minoxidil is also used off-label for hair loss treatment. High-dose oral therapy (e.g., 10 mg or more) is reserved for the treatment of severe hypertension and requires medical supervision.

Side Effects

Topical Application

  • Local skin irritation, itching, or flaking
  • Unwanted facial hair growth (especially in women)
  • Initial shedding phase at the start of therapy (shed effect), which typically resolves within a few weeks

Oral Application

  • Fluid retention (water retention, edema)
  • Increased heart rate (tachycardia)
  • Drop in blood pressure
  • Hypertrichosis (excessive body hair growth)

Important Notes

Minoxidil must be used continuously to maintain results. If treatment is discontinued, hair loss typically returns within a few months. Minoxidil should not be used during pregnancy or breastfeeding. Medical consultation is recommended before starting therapy.
