Definition and Anatomy

The nervus stapedius is a small motor nerve that arises as a branch of the facial nerve (cranial nerve VII). It travels through the facial canal within the petrous part of the temporal bone and innervates the stapedius muscle, the smallest skeletal muscle in the human body. This muscle attaches to the stapes (stirrup), one of the three ossicles in the middle ear.

Function

The primary function of the nervus stapedius is to mediate the stapedius reflex (also known as the acoustic reflex). When exposed to loud sounds, the stapedius muscle contracts reflexively, stiffening the ossicular chain. This increases impedance in the middle ear and reduces the transmission of sound vibrations to the inner ear. In this way, the nerve protects the delicate inner ear from damage caused by excessive noise.

  • Protection of the inner ear from loud sound (noise-protective reflex)
  • Dampening of self-generated sounds (e.g., during chewing or speaking)
  • Involvement in acoustic signal processing

Clinical Relevance

The nervus stapedius holds significant clinical importance, particularly in conditions affecting the facial nerve. A facial nerve palsy can involve the nervus stapedius depending on the level of injury. This manifests as hyperacusis -- an abnormal sensitivity to everyday sounds -- because the protective stapedius reflex is lost.

Diagnostic Testing via Stapedius Reflex Measurement

In clinical diagnostics, the function of the nervus stapedius is assessed using stapedius reflex testing (tympanometry with reflex measurement). This examination provides important information about:

  • The localization of a facial nerve palsy (pre- or post-ganglionic)
  • Middle ear and ossicular chain function
  • Possible brainstem or inner ear pathology

Symptoms of Nerve Dysfunction

Dysfunction or loss of the nervus stapedius can cause the following symptoms:

  • Hyperacusis: abnormal sensitivity to ordinary sounds
  • Absent stapedius reflex on audiological testing
  • Associated features of facial nerve palsy (e.g., incomplete eye closure, taste disturbance)

Course and Relationship to the Facial Nerve

The nervus stapedius branches off from the facial nerve within the facial canal at the level of the second genu. This anatomical location is diagnostically relevant: absence of the stapedius reflex alongside facial nerve palsy suggests a lesion proximal to the origin of the nervus stapedius, within the mastoid segment or more centrally. If the reflex is preserved, the lesion is located more distally.
