What is Novalgin?

Novalgin is a prescription medication containing the active ingredient metamizole sodium (also known as novaminsulfone or dipyrone). It belongs to the pyrazolone derivative class and is used as a potent analgesic and antipyretic agent. Novalgin is typically reserved for pain that does not respond adequately to other standard painkillers.

Indications

Novalgin is indicated for the following conditions:

  • Severe acute pain (e.g., post-operative or trauma-related pain)
  • Colic pain (e.g., renal or biliary colic)
  • Cancer pain in palliative care settings
  • High fever that is unresponsive to other antipyretics
  • Chronic pain when other analgesics are contraindicated or ineffective

Mechanism of Action

Metamizole, the active ingredient in Novalgin, inhibits the synthesis of prostaglandins in both the central and peripheral nervous systems. Prostaglandins are signaling molecules that mediate pain, inflammation, and fever. By reducing prostaglandin production, Novalgin lowers pain sensitivity and body temperature. Additionally, metamizole exerts a direct antispasmodic effect on smooth muscle, making it particularly effective for colic-type pain.

Dosage and Administration

Novalgin is available in several formulations:

  • Tablets (500 mg metamizole)
  • Oral drops (500 mg/ml solution)
  • Suppositories (available in various strengths for adults and children)
  • Injection solution (for clinical use)

The standard adult dose is 500 mg to 1000 mg per administration, up to 4 times daily. The maximum daily dose is 4000 mg. Dosage should always be determined by a qualified healthcare professional.

Side Effects

Novalgin is generally well tolerated, but in rare cases, serious side effects may occur:

  • Agranulocytosis: A rare but potentially life-threatening reduction in white blood cells. Patients should seek immediate medical attention if they develop fever, sore throat, or mouth sores during treatment.
  • Allergic reactions: Skin rash, itching, breathing difficulties, or in severe cases, anaphylactic shock.
  • Drop in blood pressure: Particularly with intravenous administration.
  • Gastrointestinal discomfort: Nausea or stomach upset in rare cases.

Contraindications

Novalgin must not be used in patients with:

  • Known hypersensitivity to metamizole or other pyrazolone derivatives
  • Impaired bone marrow function
  • Certain conditions such as hereditary glucose-6-phosphate dehydrogenase deficiency
  • During the first and third trimesters of pregnancy, or while breastfeeding
  • Infants under 3 months of age or weighing less than 5 kg

Important Notes

Alcohol should be avoided while taking Novalgin. Long-term use requires regular monitoring of blood counts. Novalgin may reduce the effectiveness of ciclosporin (an immunosuppressant) and may enhance the effect of warfarin (an anticoagulant). Patients should inform their doctor about all other medications they are taking to avoid interactions.
