What is Proctology?

Proctology is a medical specialty concerned with the diagnosis and treatment of diseases affecting the rectum, the anus, and the surrounding pelvic floor tissue. It is closely related to general surgery and gastroenterology. Physicians who specialize in this field are called proctologists or colorectal surgeons.

Proctological conditions are very common in the general population but are often not treated promptly due to embarrassment or social stigma. However, many conditions in this area respond well to treatment when diagnosed early.

Common Conditions in Proctology

The specialty covers a wide range of conditions, including:

  • Hemorrhoids: Enlarged vascular cushions in the anal canal that can cause itching, bleeding, and a feeling of pressure.
  • Anal fissures: Tears in the lining of the anus that cause significant pain during bowel movements.
  • Anal fistulas and abscesses: Inflammatory conditions near the anus, often resulting from infections of the anal glands.
  • Rectocele: A bulge of the rectal wall toward the vagina or perineum, commonly associated with pelvic floor weakness.
  • Rectal prolapse: Protrusion of part of the rectum through the anus.
  • Fecal incontinence: Involuntary leakage of stool or gas due to weakness of the sphincter muscle.
  • Inflammatory bowel disease (IBD): Particularly Crohn's disease, which frequently involves the anal region.
  • Anal eczema: An inflammatory skin condition of the anal area causing itching and skin changes.
  • Rectal and anal cancer: Malignant tumors of the rectum and anal canal.

Diagnosis in Proctology

A proctological evaluation typically begins with a thorough medical history and a physical examination. Key diagnostic methods include:

  • Visual inspection: Assessment of the anal area for skin changes, fistula openings, or hemorrhoids.
  • Digital rectal examination (DRE): Manual palpation of the rectum to assess sphincter tone, the prostate, or palpable abnormalities.
  • Proctoscopy: Endoscopic examination of the anal canal and lower rectum using a rigid or flexible instrument.
  • Rectoscopy: Extended internal examination of the rectum up to approximately 25 cm in depth.
  • Colonoscopy: Full endoscopic examination of the colon, used when more extensive disease is suspected.
  • Endosonography: Ultrasound examination performed from within the anal canal to assess the sphincter and surrounding structures.
  • Manometry: Measurement of pressure within the anal canal to evaluate sphincter function.

Treatment in Proctology

Treatment options vary depending on the type and severity of the condition:

Conservative Treatment

Many proctological conditions can initially be managed without surgery, including:

  • Stool regulation through a high-fiber diet and adequate fluid intake
  • Topical medications such as ointments and suppositories to relieve inflammation and pain
  • Sitz baths to promote hygiene and sphincter relaxation
  • Pelvic floor exercises to strengthen the surrounding musculature
  • Biofeedback therapy for fecal incontinence

Minimally Invasive and Surgical Treatment

For more advanced conditions, the following procedures may be used:

  • Sclerotherapy: Injection of a sclerosing agent into hemorrhoids to reduce blood supply.
  • Rubber band ligation: Placement of elastic bands around hemorrhoidal tissue to cut off its blood supply.
  • Hemorrhoidectomy: Surgical removal of hemorrhoids.
  • Fistula surgery: Surgical treatment of anal fistulas, tailored to the complexity of the fistula tract.
  • Sphincter reconstruction: Repair of the anal sphincter in cases of incontinence.
  • Rectal resection: Surgical removal of part of the rectum for cancer or severe prolapse.

When to See a Proctologist

Medical evaluation is recommended if you experience:

  • Blood on stool or on toilet paper
  • Pain, burning, or itching in the anal area
  • Changes in bowel habits such as diarrhea, constipation, or painful defecation
  • Mucus discharge or involuntary loss of stool
  • Palpable lumps or protrusions near the anus
  • Persistent inflammation or fistula openings in the anal region

While many of these symptoms point to benign conditions, they may in some cases indicate more serious diseases such as colorectal cancer. Early medical evaluation is therefore strongly recommended.
