What is Prostate Inflammation?

Prostate inflammation, medically referred to as prostatitis, is an inflammatory condition affecting the prostate gland. The prostate is a small, walnut-sized gland located below the bladder in men and plays a key role in the production of seminal fluid. Prostatitis is one of the most common urological conditions in men under the age of 50.

There are four recognized categories:

  • Acute bacterial prostatitis: sudden onset bacterial infection of the prostate
  • Chronic bacterial prostatitis: recurring bacterial infection with milder symptoms
  • Chronic pelvic pain syndrome (CPPS): the most common form, non-infectious, often with unclear cause
  • Asymptomatic inflammatory prostatitis: no symptoms, usually discovered incidentally

Causes

The causes of prostatitis depend on the specific type:

  • Bacterial forms: commonly caused by Escherichia coli, enterococci, or other gram-negative bacteria entering the prostate via the urethra or bloodstream
  • Chronic pelvic pain syndrome: possible causes include pelvic floor muscle tension, autoimmune reactions, psychological stress, or nerve dysfunction
  • Contributing factors: urinary tract infections, urinary catheters, urethral strictures, and a weakened immune system

Symptoms

Symptoms vary considerably depending on the type of prostatitis:

Acute Prostatitis

  • Sudden high fever and chills
  • Severe pain in the pelvis, perineum, or lower back
  • Painful and difficult urination
  • Frequent urge to urinate, including at night
  • General feeling of illness

Chronic Prostatitis / Pelvic Pain Syndrome

  • Persistent or recurring pain in the lower abdomen, perineum, or genital area
  • Burning sensation during urination
  • Pain during ejaculation
  • Erectile dysfunction and reduced quality of life
  • Absent or low-grade fever

Diagnosis

Several examinations are used to diagnose prostatitis:

  • Medical history and physical examination: symptom assessment and digital rectal examination (DRE) of the prostate
  • Urinalysis: detection of bacteria, white blood cells, or signs of inflammation in the urine
  • Blood tests: elevated inflammatory markers (CRP, leukocytes), and possibly PSA (prostate-specific antigen) levels
  • Prostatic massage and secretion analysis: to differentiate bacterial from non-bacterial forms
  • Ultrasound: transrectal ultrasound to evaluate the prostate gland

Treatment

Bacterial Prostatitis

Bacterial prostatitis is primarily treated with antibiotics that penetrate prostate tissue effectively. Commonly used agents include fluoroquinolones (e.g., ciprofloxacin) or trimethoprim-sulfamethoxazole. Treatment duration is typically 4 to 6 weeks. Severe acute prostatitis may require hospitalization with intravenous antibiotics.

Chronic Pelvic Pain Syndrome

Since no bacteria are detectable, treatment is more individualized and multifaceted:

  • Anti-inflammatory medications (NSAIDs) for pain relief
  • Alpha-blockers to relax bladder neck muscles
  • Physiotherapy and pelvic floor rehabilitation
  • Psychological support and stress management
  • Heat therapy applied to the pelvic region
  • In some cases, herbal remedies (e.g., quercetin, saw palmetto extract)

Prognosis and Outlook

Acute bacterial prostatitis typically resolves completely with timely antibiotic treatment. Chronic pelvic pain syndrome is more challenging to manage and may persist for months or years. A consistent multimodal treatment approach can significantly improve quality of life. Repeated infections should prompt investigation of contributing or underlying risk factors.
